摘要
目的探讨剖宫产与无痛分娩对产妇产后盆底功能的影响。方法选取2018年1月至12月于本院分娩的98例初产妇患者,分为剖宫产组(n=49)、无痛分娩组(n=49)。剖宫产组实施剖宫产手术,无痛分娩组实施自控泵止痛分娩,比较不同分娩方式对产妇分娩3月后盆地肌肉张力、尿道功能指标以及性生活质量的影响。结果无痛分娩组阴道静吸压(VRP)(31.66±6.11)cmH2O(1cmH2O=98 Pa)、阴道收缩压(VSP)(29.64±6.97)cmH2O、阴道收缩持续时间(4.67±1.11)s、功能性尿道长度(29.64±6.46)mm、最大尿道关闭压(30.78±6.91)cmH2O,均好于剖宫产组,差异有统计学意义(P<0.05)。剖宫产组分娩3月后恢复性生活率36.73%,性交疼痛率4.08%,性欲下降率10.20%,性生活中尿失禁率6.12%,性高潮障碍8.16%,均低于无痛分娩组,差异有统计学意义(均P<0.05)。结论相比剖宫产分娩方式,无痛分娩对盆底功能损伤更小。剖宫产除分娩3月后性生活恢复率低于无痛分娩组,对其他性生活质量指标相对而言影响较小,因此需结合临床指标,选择合适的分娩方式。
objective To study the effects of cesarean section and painless delivery on postpartum pelvic floor function.Methods 98 primiparae delivered in our hospital from January,2018 to December,2018,were selected,and were divided into a cesarean section group and a painless delivery group,with 49 in each group.The muscle tension,urethral function,and sexual quality 3 months after delivery were compared between the two groups.Results The vaginal suction pressure,vaginal contraction pressure,duration of vaginal contraction,the length of functional urethra,and maximum urethral closure pressure were(31.66±6.11)cmH2O(1cmH2O=98 Pa),(29.64±6.97)cmH2O,(4.67±1.11)s,(29.64±6.46)mm,and(30.78±6.91)cmH2O in the painless delivery group,which were better than those in the cesarean section group(all P<0.05).The rates of sexual life recovery,sexual intercourse pain,sexual desire decline,urinary incontinence during sexual life,orgasm disorder were 36.73%,4.08%,10.20%,6.12%,and 8.16%in the cesarean section group,which were lower than those in the painless delivery group,with statistical differences(all P<0.05).Conclusion Compared with cesarean delivery,painless delivery has less damage to pelvic floor function.3 months after delivery,the recovery rate of sexual life in the women taking cesarean section is lower than that in those taking painless delivery,but cesarean section has little effect on other sexual quality indicators,so it is necessary to choose a appropriate mode of delivery in according to their clinical indicators.
