摘要
目的探讨血必净注射液对严重脓毒症心肌抑制患者的疗效及对血清生化指标的影响。方法60例严重脓毒症心肌抑制患者,按照随机原则分为对照组和治疗组,各30例。两组患者均给予常规治疗,对照组在常规治疗基础上仅给予等量的生理盐水治疗,治疗组在常规治疗基础上给予血必净注射液治疗。观察比较两组患者治疗前后心肌损伤标志物[心肌肌钙蛋白I(cTnI)、肌酸激酶同工酶(CK-MB)、脑利钠肽(BNP)、C反应蛋白(CRP)]水平以及血清生化指标[白细胞计数(WBC)、血浆谷丙转氨酶(ALT)、谷草转氨酶(AST)、尿素氮(BUN)]水平。结果治疗后,两组患者cTnI、CKMB、BNP、CRP水平均较本组治疗前明显降低,且治疗组患者cTnI(0.92±0.36)μg/L、CK-MB(22.48±7.32)IU/L、BNP(1569.4±837.3)ng/L、CRP(10.4±3.5)mg/L均低于对照组的(1.15±0.41)μg/L、(28.57±6.98)IU/L、(2065.8±976.7)ng/L、(14.5±4.2)mg/L,差异均有统计学意义(P<0.05)。治疗后,两组患者WBC、ALT、AST、BUN水平均低于本组治疗前,且治疗组患者WBC(8.9±2.6)×10^9/L、ALT(110.7±19.8)IU/L、AST(127.8±27.4)IU/L、BUN(8.3±2.5)mmol/L均明显低于对照组的(12.8±3.2)×10^9/L、(205.9±21.3)IU/L、(199.7±28.7)IU/L、(9.9±3.2)mmol/L,差异均有统计学意义(P<0.05)。结论通过对严重脓毒症心肌抑制患者心肌损伤标志物的监测,证实了血必净注射液对其心肌损伤具有一定的保护作用,同时还能够使患者的肝肾功能、心肌酶等生化指标得到较好的恢复,临床治疗效果较佳,在医院值得进一步推广。
Objective To discuss the effect of Xuebijing injection on myocardial depression in severe sepsis and its influence on serum biochemical indexes.Methods A total of 60 cases of severe sepsis with myocardial inhibition were randomly divided into control group and treatment group,with 30 cases in each group.Both groups received conventional therapy,and the control group received same amount of normal saline based on the conventional treatment,and the treatment group received Xuebijing injection based on the conventional treatment.The myocardial injury markers[cardiac troponin I(cTnI),creatine kinase isoenzyme(CK-MB),brain natriuretic peptide(BNP),C-reactive protein(CRP)]levels and serum biochemical indexes[white blood cell(WBC)count,plasma alanine aminotransferase(ALT),aspartate aminotransferase(AST),blood urea nitrogen(BUN)]levels before and after treatment were observed and compared between the two groups.Results After treatment,the levels of cTnI,CK-MB,BNP and CRP of the two groups were obviously lower than those before treatment,and cTnI(0.92±0.36)μg/L,CK-MB(22.48±7.32)IU/L,BNP(1569.4±837.3)ng/L,CRP(10.4±3.5)mg/L of the treatment group were all lower than those of the control group(1.15±0.41)μg/L,(28.57±6.98)IU/L,(2065.8±976.7)ng/L,(14.5±4.2)mg/L,and the difference was statistically significant(P<0.05).After treatment,the levels of WBC,ALT,AST and BUN of the two groups were lower than those before treatment,and WBC(8.9±2.6)×10^9/L,ALT(110.7±19.8)IU/L,AST(127.8±27.4)IU/L,BUN(8.3±2.5)mmol/L of the treatment group were obviously lower than those of the control group(12.8±3.2)×10^9/L、(205.9±21.3)IU/L,(199.7±28.7)IU/L,(9.9±3.2)mmol/L,and the difference was statistically significant(P<0.05).Conclusion By monitoring myocardial injury markers in patients with severe sepsis,it has been confirmed that Xuebijing injection has a certain protective effect on myocardial injury,and it can also recover the biochemical indexes such as liver and kidney function,myocardial enzyme and so on.The clinical treatme
