摘要
目的探讨足部损伤皮肤软组织缺损伴骨关节外露创面的修复方法与效果。方法2010年1月~2019年12月,我们采用负压引流技术结合小腿及足背部皮瓣修复足部损伤骨关节外露创面36例。结果33例皮瓣全部成活,2例采用逆行腓肠神经滋养血管皮瓣,1例逆行隐神经滋养血管皮瓣,修复足背中段创面,皮瓣远端2cm坏死,经换药,创面肉芽生长,植皮后创面愈合。随访3月~2年,皮瓣外形满意,足功能恢复良好。结论负压封闭引流技术联合皮瓣修复足部皮肤缺损伴骨关节外露是一种较好治疗方法,具有操作简单,皮瓣成活率高,不损伤主要血管,足功能恢复满意,适合在基层医院开展。
Objective To explore the method and effect of repairing the skin and soft tissue defect with bone and joint exposure.Methods The technique of negative pressure drainage combined with the skin flap of the lower leg and the back of the foot to repair 36 cases of bone and joint exposed wounds was used from January 2010 to December 2019. Results All 33 flaps were survived. 2 cases were treated with retrograde sural neurovascular flap and 1 case with retrograde saphenous neurovascular flap to repair the middle part of the dorsum of the foot. The distal part of the flap was 2 cm necrotic. After dressing change, the granulation of the wound grew and the wound healed after skin grafting. The follow-up of 3 months to 2 years showed that the shape of flap was satisfactory and the function of foot recovered well. Conclusions The technique of vacuum sealing and drainage combined with skin flap to repair the skin defect of foot with bone and joint exposure is a better treatment method, which has the advantages of simple operation, high survival rate of skin flap, no damage to the main blood vessels, satisfactory recovery of foot function, and is suitable for basic hospital.
