摘要
目的观察热敏灸联合西药干预冠心病合并左心功能不全的临床疗效。方法将70例冠心病合并左心功能不全患者随机分为对照组和观察组,每组35例。对照组予常规西药治疗,观察组在此基础上予热敏灸治疗,治疗1个月。记录两组患者治疗前后心绞痛发作频次、心绞痛持续时间及6 min步行距离;利用彩色多普勒超声心动仪测定左心室射血分数(left ventricular ejection fraction, LVEF)、左室收缩末期内径(left ventricular end systolic diameter, LVESD)及左室舒张末期内径(left ventricular end diastolic diameter,LVEDD);评定两组患者治疗前后生活质量评分,并检测两组患者治疗前后血清肿瘤坏死因子-a(tumor necrosis factor-a, TNF-a)、白介素-6(interleukin-6, IL-6)水平。结果两组治疗后心绞痛发作频次、心绞痛持续时间明显降低(P<0.05),观察组均低于对照组(P<0.05);两组治疗后6 min步行距离均明显增加(P<0.05),观察组6 min步行距离明显长于对照组(P<0.05);两组患者LVEF、LVESD及LVEDD等左心功能指标均明显改善(P<0.05),观察组优于对照组(P<0.05);观察组生活质量评分明显改善(P<0.05),且优于对照组(P<0.05);观察组患者血清TNF-a、IL-6水平明显降低(P<0.05),而对照组无明显变化(P>0.05),两组间有明显差异(P<0.05)。结论热敏灸联合西药治疗冠心病合并左心功能不全可明显缓解冠心病心绞痛症状,改善患者左心功能,提升患者生活质量。这可能与其降低患者血清TNF-a、IL-6水平有关。
Objective To observe the clinical efficacy of heat-sensitive moxibustion combined with Western medication in treating coronary heart disease(CHD)coupled with left heart dysfunction.Method Seventy patients with CHD coupled with left heart dysfunction were randomized into a control group and an observation group,with 35 cases in each group.The control group was given routine Western medications for treatment,and the observation group was additionally given heat-sensitive moxibustion.The treatment lasted for 1 month.The angina attack frequency and duration and 6-min walk distance were recorded in the two groups before and after treatment;the left ventricular ejection fraction(LVEF),left ventricular end-systolic diameter(LVESD)and left ventricular end-diastolic diameter(LVEDD)were measured using color Doppler echocardiography;the quality of life(QOL)score was evaluated before and after treatment,and the serum levels of tumor necrosis factor-a(TNF-a)and interleukin-6(IL-6)were detected before and after treatment.Result The angina frequency and duration decreased significantly after treatment in the two groups(P<0.05),and were lower in the observation group than in the control group(P<0.05);the 6-min walk distance increased significantly after treatment in both groups(P<0.05),and was markedly longer in the observation group than in the control group(P<0.05);the left heart function indexes such as LVEF,LVESD and LVEDD showed significant improvements in both groups(P<0.05),and were better in the observation group than in the control group(P<0.05);the QOL score showed significant improvement in the observation group(P<0.05),and was superior to that in the control group(P<0.05);the serum levels of TNF-aand IL-6 declined significantly in the observation group(P<0.05)but did not show significant changes in the control group(P>0.05),and the between-group differences were statistically significant(P<0.05).Conclusion Heat-sensitive moxibustion combined with Western medication can notably mitigate angina,improve the left heart f
