摘要
目的探讨改良冲管在血浆置换治疗中的应用价值。方法选取2016年3月至2019年3月在我院行单重血浆置换的重型肝炎患者88例为研究对象,根据冲管方法不同分为观察组和对照组,对照组采用传统冲管,观察组采用改良冲管,比较两组血浆滤过器和管路凝血分级、血浆置换前后的凝血酶原时间(PT)、活化部分凝血活酶时间(APTT),滤过器滤过速度、血浆置换总时间、血浆置换前后的谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆红素(TBil)水平以及血浆置换过程中不良反应发生情况。结果观察组血浆滤过器和管路凝血分级优于对照组,血浆置换后PT,APTT短于对照组,血浆滤过速度、血浆置换总时间优于对照组,各项肝功能指标均优于对照组(P<0.05),不良反应发生情况少于对照组(P<0.05)。结论在重症肝炎血浆置换治疗过程中,改良冲管血浆置换效果好,可明显改善肝功能,不良反应发生率低,具有较高的临床应用价值。
Objective To explore the application value of modified washing tube in therapy with plasma exchange.Methods To select 88 cases of patients with severe hepatitis who underwent single plasma exchange in our hospital from March 2016 to March 2019 as the study objects,and they were divided into an observation group and a control group according to different methods of watering tube.The control group used traditional watering tube,and the observation group used modified washing tube to compare the coagulation grades of plasma filters and flow lines,prothrombin time(PT)and activated partial thromboplastin time(APTT)before and after plasma exchange,filtration rate of filters,total time of plasma exchange,alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TBil)levels before and after plasma exchange,and the occurrence of adverse reactions during the replacement process.Results The coagulation grading of the plasma filter and the flow lines in the observation group was better than those in the control group,the PT and APTT after plasma exchange were shorter than those in the control group,the filtration rate of plasma and total time of plasma replacement were better than those of the control group,and all indexes of liver function of the observation group were better than those of the control group(P<0.05).The occurrence of adverse reactions in the observation group was less than that in the control group(P<0.05).Conclusion In the process of plasma exchange therapy for severe hepatitis,the plasma exchange effect by modified washing tube was good,it can significantly improve liver function,and the incidence rate of adverse reactions was low.Therefore,it has higher clinical application value.
