摘要
目的:探讨血清尿素(Urea)、肌酐(Creat)、胱抑素C(CysC)及中性粒细胞明胶酶相关脂质运载蛋白(NGAL)检测在早期诊断糖尿病肾病中的应用价值。方法:将2019年1月至6月期间绵阳市中心医院收治的78例2型糖尿病患者和同期在该医院进行健康体检的40例健康人作为研究对象。依据这78例2型糖尿病患者尿微量白蛋白与肌酐的比值(ACR)将其分为A组(尿微量白蛋白正常组)和B组(微量白蛋白尿组)。将这40例健康人作为对照组。对三组受检者均进行血清Urea、Creat、CysC及NGAL水平的检测,对比分析其检测的结果。结果:A组和B组患者血清Urea、CysC、Creat及NGAL的水平均高于对照组受检者。A组患者与对照组受检者血清Urea、CysC和NGAL的水平相比,P>0.05。B组患者与对照组受检者血清Urea的水平相比,P<0.05。A组、B组和对照组受检者血清Creat的水平相比,P>0.05。B组患者血清CysC和NGAL的水平与A组患者、对照组受检者相比,P<0.05。经ROC曲线分析,血清NGAL在ROC曲线下的面积最大。结论:在糖尿病肾病患者出现病理性蛋白尿之前,其血清CysC和NGAL的水平就开始升高。在糖尿病肾病患者出现微量白蛋白尿以后,其血清CysC和NGAL的水平可显著升高。血清CysC和NGAL都可作为早期诊断糖尿病肾病的敏感、可靠指标。
objective:to evaluate the application value of serum Urea,creatinine,cystatin C(CysC)and neutrophil gelatinase-associated lipid carrier protein(NGAL)in the early diagnosis of diabetic nephropathy.Methods:118 patients with type 2 diabetes admitted to mianyang central hospital from January to June 2019 and 40 healthy people who underwent physical examination in the hospital during the same period were studied.Based on the ratio of microalbumin to creatinine(ACR),the 118 patients with type 2 diabetes were divided into group A(normal microalbumin)and group B(microalbuminuria).These 40 healthy people were used as the control group.Serum levels of trainees,Creat,CysC and NGAL were tested in all three groups,and the test results were compared and analyzed.Results:serum levels of serum,CysC,Creat and NGAL in group A and group B were higher than those in the control group.The serum levels of serum,CysC and NGAL in patients in group A were compared with those in the control group,and P>0.05.Compared the serum levels of patients in group B with those in the control group,P<0.05.Serum Creat levels in group A,group B and control group were P>0.05.The serum levels of CysC and NGAL in patients in group B were compared with those in group A and control group(P<0.05).According to ROC curve analysis,the area of serum NGAL under ROC curve was the largest.Conclusions:serum levels of CysC and NGAL were elevated in diabetic nephropathy patients prior to the onset of pathologic proteinuria.Serum levels of CysC and NGAL were significantly increased in diabetic nephropathy patients with microalbuminuria.Serum CysC and NGAL can be used as sensitive and reliable indicators for early diagnosis of diabetic nephropathy.
