虚拟现实技术结合任务导向性训练对脑卒中偏瘫患者上肢功能的影响 被引量:16

摘要 目的 探讨脑卒中偏瘫患者运用虚拟现实技术结合任务导向性训练对上肢功能的影响.方法 选取2017年1月~2019年6月我院收治的60例脑卒中偏瘫患者进行实验研究,根据不同的治疗方法分为基础组(n=30)和结合组(n=30),基础组患者采用常规的康复训练,结合组患者给予虚拟现实技术结合任务导向性训练.两组在治疗前和治疗8周后使用改良Ashworth痉挛量表、Fugl-Meyer评定量表中上肢部分(FMA-UE)、改良Barthel指数(BMI)及上肢关节活动范围进行评估,比较两组的疗效差异.结果 两组治疗后患侧肢体肌肉痉挛评分低于治疗前,差异有统计学意义(P<0.05);两组治疗后上肢功能、生活能力评分均高于治疗前,差异有统计学意义(P<0.05).结合组治疗后患侧肢体肌肉痉挛评分低于基础组,差异有统计学意义(P<0.05);结合组治疗后上肢功能、生活能力评分均高于基础组,差异有统计学意义(P<0.05);结合组治疗前肩关节及肘关节活动角度与基础组比较,差异无统计学意义(P>0.05);结合组、基础组治疗后肩关节、肘关节活动角度均大于治疗前,差异有统计学意义(P<0.05);结合组治疗后肩关节、肘关节活动角度均大于基础组,差异有统计学意义(P<0.05).结论 脑卒中偏瘫患者运用虚拟现实技术结合任务导向性训练可有效改善患者上肢功能,恢复活动范围,缓解肌肉痉挛,促进日常生活能力恢复. Objective To explore the effect of virtual reality technology combined with task-oriented training on upper extremity function of cerebral vascular accident(CVA)patients with hemiplegia.Methods 60 CVA patients with hemiplegia admitted to our hospital from January 2017 to June 2019 were randomly selected for experimental study.According to different treatment methods,the 60 patients were divided into the basic group(n=30)and the combination group(n=30).Patients in the basic group were given conventional rehabilitation training,while patients in the combination group were given virtual reality technology combined with task-oriented training.Before treatment and after 8 weeks of treatment,the two groups were evaluated with modified Ashworth spasticity scale,Fugl-Meyer index of upper extremity(FMA-UE),modified Barthel Index(BMI)and upper extremity joint range of motion to compare the difference of efficacy between the two groups.Results The scores of affected extremity muscle spasm after treatment in the two groups were lower than those before treatment,and the differences were statistically significant(P<0.05).After treatment,the scores of upper extremity function and life ability in the two groups were higher than those before treatment,and the difference was statistically significant(P<0.05).The scores of affected extremity muscle spasm after treatment in the combination group was lower than that in the basic group,and the difference was statistically significant(P<0.05).After treatment,the scores of upper extremity function and life ability of the combined group were higher than those of the basic group,and the differences between the groups were statistically significant(P<0.05).There was no statistically significant difference in shoulder joint and elbow joint movement angle between the combined group and the basic group before treatment(P>0.05).The movement angles of shoulder joint and elbow joint after treatment in the combination group and the basic group were all larger than those before treatment,and the d
