摘要
目的研究细胞外组蛋白在急性呼吸窘迫综合征(ARDS)发病过程中的变化,探索其具体作用及临床价值。方法通过氯气染毒复制小鼠ARDS模型,在不同浓度氯气染毒条件下,检测血浆中细胞外组蛋白H4、血管内皮细胞损伤标志物sTM、肺组织病理的改变;分析组蛋白H4与sTM的关系。结果正常小鼠血浆中组蛋白H4的平均浓度为(076±017)μg/ml;氯气染毒后,小鼠血浆中组蛋白H4水平升高;当200 ppm氯气染毒(30min)后12 h,血浆中组蛋白H4的平均浓度为(768±247)μg/ml。氯气浓度越高,血浆中组蛋白升高越明显,二者具有明显的正相关性。在氯气吸入损伤中,血浆中内皮损伤标志物sTM也明显升高,且与血浆中组蛋白H4具有明显的相关性(r=08302)。HE染色镜下可见,染毒小鼠肺组织炎症细胞浸润,肺泡间隔增宽,肺泡上皮细胞和血管内皮细胞肿胀,且随氯气浓度增高损伤加重。结论细胞外组蛋白在氯气所致ARDS小鼠血浆中明显升高,且与肺损伤程度密切相关,提示其可能在ARDS发病机制中起重要作用。
Objective To study the changes of extracellular histones in acute respiratory distress syndrome(ARDS),thereby explore its specific rule and clinical value ARDS.Methods The ARDS model of mice was replicated by chlorine gas in-halation.The changes of extracellular histone H4,endothelial injury marker sTM in plasma and lung histopathology under differ-ent concentrations of chlorine gas exposure were detected,and make further analysis on the relationship between histone H4 andsTM.Results The results showed that the mean concentration of plasma histone H4 in control mice was(0.76±0.17)μg/ml,which was rised after chlorine gas exposure,the mean concentration of plasma histone H4 was(7.68±2.47)μg/ml 12 h after200 ppm of chlorine gas exposure for 30 min;the higher the concentration of chlorine gas was,the more obvious the increase ofhistone in plasma,there was a positive correlation between them.Additionally,the endothelial injury marker sTM in plasma wasalso significantly increased during chlorine gas inhalation,which also showed a significant correlation with plasma level of histoneH4(coefficientr=0.8302).Through HE staining,infiltration of inflammatory cells,widening of alveolar septum,andswelling of alveolar epithelial cells and vescular endothelial cells were obvious under a microscope in chlorine gas-treated mice,which were aggravated with the increase of chlorine gas concentration.Conclusion The results showed that extracellularhistones in plasma was significantly increased during chlorine gas-induced ARDS in mice,and closely related to the degree of in-jury,that suggested that this kind of protein might play an important role in the pathogenesis of ARDS.
