摘要
目的研究新生儿败血症血清25羟维生素D[25(OH)D]与免疫球蛋白水平变化及意义。方法将我院2016年3月至2018年3月期间收治的50例新生儿败血症患儿分为普通败血症组(n=34)与严重败血症组(n=16),同时纳入30例健康新生儿作为对照组,①比较败血症患儿与健康对照组间相关血生化指标;②比较普通败血症组与严重败血症组血清25(OH)D与IgM、IgG水平;③比较治疗前后及不同治疗效果败血症患儿血清25(OH)D、IgM、IgG水平;④分析败血症患儿血清25(OH)D、IgM、IgG水平与WBC、PLT、CRP水平间的相关性;⑤绘制ROC曲线,分析血清25(OH)D、IgM、IgG水平在诊断新生儿败血症中的价值。结果败血症患儿PLT、血清25(OH)D、IgG水平明显低于健康对照组,IgM、CRP、WBC水平明显高于对照组(P<0.05);普通败血症组患儿血清25(OH)D、IgG水平显著高于严重败血症组(P<0.05),IgM水平显著低于严重败血症组(P<0.05);败血症组患儿经治疗后血清25(OH)D水平较治疗前显著上升(P<0.05),IgM水平显著下降(P<0.05),IgG差异无统计学意义(P>0.05),且血清25(OH)D、IgG水平在痊愈组、好转组及无效组患儿中依次降低(P<0.05),IgM水平在痊愈组、好转组及无效组患儿中依次升高(P<0.05);相关性分析提示,败血症患儿血清25(OH)D水平与其PLT水平呈正相关(r=0.53,P<0.05),与WBC、CRP水平呈负相关(r=-0.44,-0.46,P<0.05),IgG与CRP水平呈负相关(r=-0.35,P<0.05);ROC曲线提示,血清25(OH)D、IgM、IgG及血清25(OH)D+IgM+IgG在诊断新生儿败血症中具有良好应用价值。结论新生儿败血症患儿血清25(OH)D、IgG、IgM能有效提示患儿病情,在诊断新生儿败血症中也具有一定的应用价值。
Objective To study changes and significances of serum 25-hydroxyvitamin D[25(OH)D]and immunoglobulins in neonatal sepsis.Methods According to the relevant criteria,50 cases of pediatric patients with neonatal sepsis admitted to our hospital from March,2016 to March,2018 were divided into the common sepsis group(n=34)and the severe sepsis group(n=16).And 30 cases of healthy newborns were selected as the control group.The related blood biochemical parameters were compared between septic pediatric patients and the healthy control group.The levels of 25(OH)D,IgM and IgG in peripheral blood were compared between the common sepsis group and severe sepsis group.The levels of blood 25(OH)D,IgM and IgG were compared before and after the treatment and among septic pediatric patients with different treatment effects.The correlation between levels of peripheral blood 25(OH)D,IgM and IgG and WBC,PLT and CRP were analyzed among pediatric patients with sepsis.The ROC curve was drawn to analyze the effects of peripheral blood 25(OH)D and IgG in the diagnosis of neonatal sepsis.Results The levels of PLT and peripheral blood 25(OH)D,IgM and IgG in pediatric patients with sepsis were significantly lower than those in the healthy control group,and the levels of IgM,CRP and WBC were significantly higher than those in the control group(P<0.05).The levels of serum 25(OH)D and IgG in the common sepsis group were significantly higher than those in the severe sepsis group(P<0.05),while the IgM level was significantly lower than that in the severe sepsis group(P<0.05).The level of serum 25(OH)D in the sepsis group after treatment was significantly increased compared with that of before treatment(P<0.05),while the IgM level was significantly decreased(P<0.05).There was no significant difference in the IgG(P>0.05),and the levels of serum 25(OH)D and IgG were decreased in turn in the cure group,effective group and ineffective group(P<0.05),while the IgM level was increased in turn in the cure group,effective group and ineffective group(P<0.05
