摘要
目的研究妊娠期亚临床甲状腺功能减退症(SCH)患者血清同型半胱氨酸(Hcy)水平与妊娠结局的关系。方法选取2013年6月至2018年6月本院收治的80例妊娠期SCH患者作为观察组,选取同期行孕期检查的甲状腺功能正常的80例孕妇作为对照组。比较两组甲状腺功能指标,血清Hcy水平和妊娠结局。根据观察组母婴是否发生不良结局将观察组孕妇分为母婴不良结局组和母婴正常结局组,比较两组血清Hcy水平。结果观察组促甲状腺激素(TSH)水平明显高于对照组(P<0.05),游离三碘甲状腺原氨酸(FT3)和游离甲状腺激素(FT4)水平与对照组无明显差异(P>0.05),Hcy水平明显高于对照组(P<0.05),母婴不良妊娠结局发生率明显高于对照组(P均<0.05)。母婴不良结局组Hcy水平明显高于母婴正常结局组(P<0.05)。结论妊娠期SCH患者甲状腺功能弱,Hcy水平高,不良妊娠结局发生率高,且血清Hcy水平与妊娠结局密切相关。
Objective To study the relationship between serum homocysteine(Hcy)level and pregnancy outcome in patients with subclinical hypothyroidism(SCH)during pregnancy.Methods From June 2013 to June 2018,80 pregnant women with Sch during pregnancy were selected as the observation group,and 80 pregnant women with normal thyroid function were selected as the control group.The thyroid function,Hcy and pregnancy outcome were compared between the two groups.According to the adverse outcome of mother and infant in the observation group,the pregnant women in the observation group were divided into two groups:the group with adverse outcome of mother and infant and the group with normal outcome of mother and infant.Results The TSH level of the observation group was significantly higher than that of the control group(P<0.05),the levels of FT3 and FT4 were not significantly different from that of the control group(P>0.05),the Hcy level was significantly higher than that of the control group(P<0.05),and the incidence of adverse pregnancy outcome was significantly higher than that of the control group(P<0.05).Hcy level in the group with adverse maternal and infant outcome was significantly higher than that in the group with normal maternal and infant outcome(P<0.05).Conclusion The thyroid function is weak,Hcy level is high and the incidence of adverse pregnancy outcome is high in the patients with Sch during pregnancy.
