摘要
目的探讨产前超声在预测胎儿肠管扩张中的应用价值。方法选取洛阳市妇幼保健院2015年3月—2018年3月收纳的62例经产前超声判断为胎儿肠管扩张患者,对其进行临床跟踪调查。结果经产前超声检查结果表明有42例孕妇出现小肠扩张,占总比例的67.75%,12例孕妇出现结肠扩张,占总比例的19.35%,8名孕妇出现十二直肠梗阻,占总比例的12.90%;有52例孕妇妊娠结局良好,排便以及经检查显示为正常新生儿,占总比例的83.87%,有10例孕妇妊娠结局不良,其中有4名选择停止妊娠进行引产,引产后经解剖证实确定为消化道畸形,有6名胎儿出生后需要进行进一步治疗;胎儿肠扩张发生比例与妊娠结局良好与否呈正比关系(P<0.05);经染色体检查发现6.45%的胎儿肠管扩张存在染色体异常现象。结论对孕妇进行产前超声检查,能够发现胎儿是否出现肠管扩张,肠管扩张严重程度与妊娠结局有着密切的关系,有利于做进一步的诊断与治疗,对预后有较大影响。
Objective To explore the value of prenatal ultrasound in predicting fetal intestinal dilatation.Methods Totally 62 patients admitted to our hospital from March 2015 to March 2018 were selected for prenatal ultrasound diagnosis of fetal intestinal dilatation,and clinical follow-up investigation was performed.Results The results of prenatal ultrasound examination showed that 42 pregnant women had dilated small intestine,accounting for 67.75%of the total,12 pregnant women had colonic dilation,accounting for 19.35%of the total,and 8 pregnant women had duodenal obstruction,accounting for 12.90%of the total.52 pregnant women had good pregnancy outcomes,and defecation and examination showed normal newborns,accounting for 83.87%of the total.There were 10 pregnant women with poor pregnancy outcomes.Among them,4 cases chose to stop pregnancy for induction of labor.After the induction of labor,anatomical confirmation was confirmed to be a malformation of the digestive tract.6 fetuses needed further treatment after birth.The proportion of fetal intestinal dilatation was proportional to whether the pregnancy outcome was good or not(P<0.05).Chromosomal examination revealed chromosomal abnormalities in 6.45%of fetal intestinal dilatations.Conclusion Prenatal ultrasound examination of pregnant women can detect whether the intestinal dilatation occurs in the fetus.The severity of intestinal dilatation is closely related to pregnancy outcome,which is conducive to further diagnosis and treatment and has a greater impact on the prognosis.
