摘要
目的观察手法推拿联合超短波治疗对慢性膝关节骨性关节炎患者疼痛及病灶超声检测指标的影响。方法将南京大学医学院附属鼓楼医院2018年1月—2019年2月诊治的124例慢性膝关节骨性关节炎患者随机分为2组,2组均给予盐酸氨基葡萄糖片治疗,对照组联合超短波治疗,观察组在对照组治疗基础上给予手法推拿治疗,连续治疗3周。观察2组临床疗效及疼痛[疼痛分级指数(PRI)、视觉模拟疼痛评分(VAS)、现在疼痛状况(PPI)]、滑膜血流超声信号、病灶超声检测指标(髌上囊积液厚度、滑膜厚度)的变化。结果观察组治疗总有效率为90.3%(56/62),对照组为72.6%(45/62),观察组明显高于对照组(P<0.05);2组治疗后PRI评分、VAS评分、PPI评分、髌上囊积液厚度及滑膜厚度均明显低于治疗前(P均<0.05),且观察组各指标均明显低于对照组(P均<0.05);2组滑膜血流信号分级均得到明显改善(P均<0.05),且观察组改善明显优于对照组(P<0.05)。结论手法推拿联合超短波治疗慢性膝关节骨性关节炎可明显缓解关节疼痛,能够有效调节滑膜血流,减轻膝关节病理变化,从而改善膝关节功能。
Objective It is to observe the effect of manipulative massage and ultrashort wave therapy on pain and lesion ultrasound indexes in patients with chronic knee osteoarthritis(KOA).Methods 124 patients with chronic KOA diagnosed and treated in Gulou Hospital from January 2018 to February 2019 were randomly divided into 2 groups,both groups were given glucosamine hydrochloride tablets,the control group was combined with ultrashort wave treatment,and the observation group was given manipulative massage based on the control group,both groups were treated for 3 weeks.The clinical efficacy and the changes of pain[pain rating index(PRI),visual visual analogue scale(VAS),present pain intensity(PPI)],ultrasound signal of synovial blood flow,and ultrasound detection index of lesions(thickness of the patellar effusion,synovial thickness)were observed in the two groups.Results The total effective rate of treatment in the observation group was 90.3%(56/62)and that in the control group was 72.6%(45/62),the observation group was significantly higher than the control group(P<0.05).After treatment,the PRI scores,VAS scores,PPI scores,the thickness of the upper patellar effusion and the thickness of the synovium were significantly lower than those before treatment in the two groups(P<0.05),and the indicators of the observation group were significantly lower than the control group(P<0.05);the synovial blood flow signal grades were significantly improved(all P<0.05),and the improvement in the observation group was significantly better than the control group(P<0.05).Conclusion Manipulative massage combined with ultrashort wave treatment can significantly relieve joint pain,effectively regulate synovial blood flow,reduce knee joint pathological changes,and thus improve knee joint function in the treatment of KOA.
