摘要
目的探讨曲张静脉抽剥术配合利湿通瘀中药湿敷治疗下肢静脉曲张伴淤积性皮炎的疗效及对近期预后的影响。方法将2016年8月—2018年8月在石家庄市中医院拟择期行手术治疗的120例下肢静脉曲张伴淤积性皮炎患者随机分为观察组和对照组,每组60例。对照组实施曲张静脉抽剥术治疗,观察组在对照组治疗基础上于术后给予利湿通瘀中药湿敷治疗,1次/d,直至术后2周。记录2组治疗前后临床症状积分、湿疹面积严重程度评分(EASI)、静脉临床严重程度评分(VCSS)、阿伯丁静脉曲张问卷(AVVQ)评分的变化,比较2组临床疗效及近期预后情况。结果2组治疗后皮肤红斑、丘疹、水肿、糜烂或溃疡、皮损区鳞屑、色素沉着、皮肤瘙痒评分和总评分均较治疗显著降低(P均<0.05),观察组治疗后以上评分均显著低于对照组(P均<0.05);2组治疗后EASI评分、VCSS评分、AVVQ评分均较治疗前显著降低(P均<0.05),观察组治疗后以上评分均显著低于对照组(P均<0.05);观察组症状总有效率显著高于对照组(P<0.05);观察组创面愈合时间、术后住院时间均显著短于对照组(P均<0.05),住院费用显著少于对照组(P<0.05),术后并发症总发生率显著低于对照组(P<0.05)。结论曲张静脉抽剥术配合利湿通瘀中药湿敷治疗下肢静脉曲张伴淤积性皮炎可明显改善临床症状,促进创面愈合,缩短住院时间,降低术后并发症发生率。
Objective It is to explore the curative effect of varicose vein stripping operation combined with Chinese medicine for disinhibiting wetness and unblocking stasis on varicose veins with stasis dermatitis of lower extremities and its impact on short-term prognosis.Methods 120 patients with varicose veins with stasis dermatitis who underwent elective surgical treatment in Shijiazhuang Hospital of TCM from August 2016 to August 2018 were randomly divided into observation group and control group,60 cases in each group.The control group was treated with varicose vein stripping,and the observation group was treated with Chinese medicine for disinhibiting wetness and unblocking stasis by wet compress on the basis of the control group,once a day until 2 weeks after the operation.The changes of clinical symptom score,eczema area severity index(EASI)score,venous clinical severity score(VCSS),and Aberdeen varicose vein questionnaire(AVVQ)were recorded before and after treatment in the two groups.The clinical efficacy and short-term prognosis of the two groups were compared.Results After treatment,the scores of erythema,papules,edema,erosion or ulceration,scaly skin lesions,pigmentation,skin itching and total score of the two groups were significantly lower than those before treatment(all P<0.05),the scores in the observation group were lower than those in the control group(all P<0.05);the EASI score,VCSS score and AVVQ score of the two groups were significantly lower than those before treatment(all P<0.05),and the above scores of the observation group after treatment were significantly lower than the control group(all P<0.05).The total effective rate of symptoms in the observation group was significantly higher than that in the control group(P<0.05);The wound healing time and postoperative hospital stay in the observation group were significantly shorter than those in the control group(P<0.05),and the hospitalization costs were significantly less than that in control group(P<0.05),the total incidence of postoperative compli
