摘要
目的:探讨使用基质辅助激光解吸电离飞行时间质谱(matrix-assisted laser desorption ionization-time of flight mass spectrometry,MALDI-TOF MS)系统(简称质谱)进行病原菌早期鉴定的临床价值。方法:收集我院2019年6月至8月血流感染、肺部感染、泌尿系统感染患者对应的血、痰及尿标本,平行使用MALDI-TOF MS(质谱鉴定)和VITEK 2 Compact全自动细菌鉴定仪(传统鉴定)进行病原菌鉴定,比较两种方法的鉴定时间及最终鉴定结果前后抗生素的调整情况。结果:MALDI-TOF MS较VITEK 2 Compact全自动细菌鉴定仪平均提前24~30 h(P<0.05);血流感染中有26名(55.3%)、肺部感染中有33名(50.8%)、泌尿系感染中有31名(43.7%)在得到最终鉴定结果后调整抗生素,质谱可更早提供抗生素调整依据。结论:质谱可早期鉴定病原菌,给疾病的诊断及治疗提供有价值的参考,利于早期精准合理的应用抗生素,对改善感染患者预后、减少抗生素的不合理使用有重要意义。
Objective:To investigate the clinical value of matrix-assisted laser desorption ionization time-of-flight mass spectrometry(MALDI-TOF MS)system for early identification of pathogenic bacteria.Methods:Blood,sputum and urine samples corresponding to the patients with blood flow infection,lung infection and urinary system infection in our hospital from June to August 2019 were collected.The pathogen was identified by MALDI-TOF MS system and VITEK 2 Compact automatic bacterial identification instrument in parallel,and the identification time of the two methods and the adjustment of antibiotics before and after the final identification results were compared.Results:MALDI-TOF MS system was 24 hours earlier than VITEK 2 Compact automatic bacterial identification instrument(P<0.05).There were 26 cases(55.3%)of blood flow infections,33 cases(50.8%)of pulmonary infections and 31 cases(43.7%)of urinary tract infections.MALDI-TOF MS system can provide the basis of antibiotic adjustment earlier after final identification.Conclusions:MALDI-TOF MS system can identify pathogenic bacteria in the early stage,provide valuable reference for the diagnosis and treatment of diseases,and is beneficial to the early accurate and reasonable use of antibiotics,which is of great significance to improve the prognosis of infected patients and reduce the unreasonable use of antibiotics.
