摘要
目的评价单向可吸收倒刺缝线单层吻合技术应用于腹腔镜胰十二指肠切除术(LPD)中的安全性及有效性。方法回顾性分析西京医院肝胆外科2016—2019年行LPD术患者80例,将其根据缝合方式分为观察组和对照组,各40例。观察组使用可吸收倒刺线完成胰肠、胆肠、胃肠单层吻合,对照组使用传统缝线及吻合技术。比较两组手术时间、术中出血量、治疗费用、住院天数、并发症发生情况及吻合时间。结果两组均成功完成腹腔镜下手术。观察组手术时间短于对照组,术中胆漏发生率低于对照组(P<0.05)。两组术中出血量、治疗费用、住院天数、术后吻合口漏发生率、吻合口狭窄发生率无显著差异(P>0.05)。结论单向可吸收倒刺缝线单层吻合技术在LPD术中应用安全有效,缩短了手术学习曲线,值得临床推广。
Objective To evaluate the safety and effectiveness of single-layer anastomosis technique with unidirectional absorbable barbed suture in laparoscopic pancreaticoduodenal surgery(LPD).Methods Eighty patients who underwent LPD from 2016 to 2019 in hepatobiliary surgery of Xijing hospital were analyzed retrospectively.The patients were divided into observation group and control group according to the suture pattern,with 40 cases in each group.In the observation group,absorbable barbed suture was used to complete single-layer anastomosis of pancreas-intestine,gallbladder-intestine and gastrointestine,traditional suture and anastomosis technology were used in the control group.The operation time,intraoperative bleeding volume,treatment cost,hospitalization days,complications and anastomotic time were compared between the two groups.Results Laparoscopic surgery was successfully completed in both groups.The operation time of the observation group was shorter than that of the control group,and the incidence of intraoperative bile leakage was lower than that of the control group(P<0.05).There were no significant differences in intraoperative bleeding volume,treatment cost,hospitalization days,incidences of postoperative anastomotic leakage and anastomotic stenosis between the two groups(P>0.05).Conclusion Single-layer anastomosis technique with unidirectional absorbable barbed suture is safe and effective in LPD,which shortens the learning curve of operation and is worthy of clinical promotion.
