摘要
杨佃会教授认为小儿遗尿多因肾气不足、膀胱虚寒、气化失司导致尿道开阖失控而发,此外,本病还与元神失养、肝失疏泄有关。杨老师常将本病分为肾气不足、肺脾气虚、心肾失交、肝经郁热四型,针灸治疗从益肾固本入手,兼顾疏肝调神,选取关元、肾俞、三阴交、中极、膀胱俞、百会、太冲为主穴,并辨证配以太溪、气海、肺俞、脾俞、足三里、神门、阴陵泉、蠡沟等穴。疗法选择以患儿为本,首选无痛之隔药灸脐法,配以毫针刺法及耳穴压豆。注重形神一体观,身体治疗与心理治疗相结合,治疗全程照顾患儿心理感受,将疏肝调神法贯穿于治疗始终。
Professor YANG Dianhui believes that the main causes of infantile enuresis is the disorder of urethra in opening and closing which results from deficiency of kidney qi,deficiency cold in bladder and inhibited qi transformation.In addition,the disease also has relationship with original spirit failing to be nourished and the liver failing to governing the free flow of qi.Professor YANG divides the disease into four syndrome types,including deficiency of kidney qi syndrome,deficiency of lung qi and spleen qi syndrome,disharmony of the heart and the kidney syndrome and heat accumulation in the liver meridian syndrome.The treatments by acupuncture and moxibustion follow the principle of tonifying kidney to strengthen the root,as well as soothing the liver and regulating the spirit.The main acupuncture points are Guanyuan(CV4),Shenshu(BL23),Sanyinjiao(SP6),Zhongji(CV3),Pangguangshu(BL28),Baihui(GV20)and Taichong(LR3).The auxiliary acupuncture points such as Taixi(KI3),Qihai(CV6),Feishu(BL3),Pishu(BL20),Zusanli(ST36),Shenmen(TF4),Yinlingquan(SP9)and Ligou(LR5)are used according to the syndrome differentiation.For the therapy,considering the characteristics of children,the painless moxibustion on medicines at the belly button is the first choice,combined with filiform needle therapy and auricular-plaster therapy.Throughout the treatment,Professor YANG pays attention to the unity of body and spirit,combines physical treatment with psychological treatment.He takes care of children’s psychological feelings and applies the method of soothing the liver and regulating the spirit.
