摘要
目的探讨分析手法整复与手术两种方法治疗踝关节骨折的应用效果。方法回顾性分析2016年6月至2018年7月本院收治的86例踝关节骨折患者的临床资料,将所选患者按照治疗方式的不同分为手术组与非手术组,每组各43例。手术组患者给予切开复位内固定术进行治疗,非手术组患者给予手法整复外固定术进行治疗,比较两组患者治疗后骨折复位疗效的优良率,并观察记录两组治疗后骨折愈合时间、住院时间及医疗费用等情况。结果治疗后,两组患者进行优良率的比较,经χ~2检验,观察组患者的优良率(93.02%)明显高于对照组患者(76.74%),对比存在统计学意义(P<0.05)。治疗后,两组患者进行骨折愈合时间、住院时间及医疗费用的比较,经t检验,观察组患者的骨折愈合时间明显低于对照组患者,但住院时间及医疗费用明显高于对照组患者,对比存在统计学意义(P<0.05)。结论切开复位内固定治疗踝关节骨折疗效显著,能最大程度的保证关节恢复正常解剖结构,缩短骨折愈合时间,而手法整复外固定治疗踝关节骨折具有操作便捷、创伤性小、医疗费用低等优点,临床应用过程中应根据患者创伤的实际情况、全身状况的好坏及患者个人意愿选择适宜的治疗方式。
Objective To explore and analyze the effect of manipulative reduction and operation in the treatment of ankle fracture.Methods The clinical data of 86 patients with ankle fracture admitted to our hospital from June 2016 to July 2018 were retrospectively analyzed.The selected patients were divided into operation group and non-operation group according to different treatment methods,43 cases in each group.The operative group was treated with open reduction and internal fixation,while the non-operative group was treated with manual reduction and external fixation.The excellent rate of fracture reduction after treatment was compared between the two groups,and the healing time,hospitalization time and medical expenses of the two groups were observed and recorded.Results After treatment,the excellent and good rate of the two groups of patients was compared.By 2 test,the excellent and good rate of the observation group(93.02%)was significantly higher than that of the control group(76.74%),and the comparison was statistically significant(P<0.05).After treatment,the fracture healing time,hospitalization time and medical expenses of the two groups were compared.T test showed that the fracture healing time of the observation group was significantly lower than that of the control group,but the hospitalization time and medical expenses were significantly higher than those of the control group,with statistical significance(P<0.05).Conclusion Open reduction and internal fixation for the treatment of ankle fracture is effective,which can ensure the restoration of normal anatomical structure and shorten the time of fracture healing.Manual reduction and external fixation for the treatment of ankle fracture has the advantages of convenient operation,less trauma and low medical cost.The clinical application should be based on the actual situation of patients’trauma,the quality of the whole body and the number of patients.People are willing to choose the appropriate treatment.
