摘要
目的观察生脉四物汤加减对糖尿病性心脏病患者血脂代谢及心血管功能指标的影响。方法将2017年1月—2019年2月首都医科大学附属北京康复医院收治的84例糖尿病性心脏病患者随机分为2组,对照组42例给予甘精胰岛素注射液皮下注射及格列齐特缓释片、阿司匹林肠溶片、阿托伐他汀钙片、盐酸贝那普利片、螺内酯片口服,观察组42例在对照组治疗基础上给予生脉四物汤加减口服,2组均连续治疗8周。对比2组治疗前后血脂代谢指标[三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)]、心血管功能指标(静息卧位心率、卧立位心率差)、超声心功能指标[左室短轴缩短率(FS)、左室射血分数(LVEF)、E/A],评估2组临床疗效,统计2组不良反应发生情况。结果2组治疗后TG、LDL-C水平均较治疗前明显降低(P均<0.05),且观察组明显低于对照组(P均<0.05);2组治疗后静息卧位心率均明显慢于治疗前(P均<0.05),且观察组明显慢于对照组(P<0.05);2组治疗后卧立位心率差及FS、LVEF、E/A均明显升高(P均<0.05),且观察组明显高于对照组(P均<0.05);观察组治疗总有效率为92.9%(39/42),对照组为73.8%(31/42),观察组明显高于对照组(P<0.05);观察组不良反应发生率为7.1%(3/42),对照组为23.8%(10/42),观察组明显低于对照组(P<0.05)。结论西医治疗基础上联合生脉四物汤加减能够明显改善糖尿病性心脏病患者血脂代谢,提高心血管功能和心脏舒张功能,提高治疗总有效率,减少西药不良反应。
Objective It is to observe the effect of modified Shengmai Siwu decoction on blood lipid metabolism and cardiovascular function indexes in patients with diabetic heart disease.Methods 84 patients with diabetic heart disease admitted to Beijing Rehabilitation Hospital from January 2017 to February 2019 were randomly divided into 2 groups,42 patients in the control group were given insulin glargine injection by subcutaneous injection and sustained release tablets of gliclazide and enteric-coated tablets of aspirin,atorvastatin calcium tablets,benazepril hydrochloride tablets,and spironolactone tablets by oral administration,42 cases in the observation group were given modified Shengmai Siwu decoction by oral administration based on the treatment of the control group,and both groups were treated continuously for 8 weeks.The blood lipid metabolism indexes[triglyceride(TG),low-density lipoprotein cholesterol(LDL-C)],cardiovascular function indexes(resting heart rate in resting position,heart rate difference in lying position),and ultrasound heart function indexes[left ventricular short axis shortening rate(FS),left ventricular ejection fraction(LVEF),E/A]before and after treatment in the two groups were compared,the clinical efficacy of the two groups was evaluated,and the occurrence of adverse reactions in the two groups was counted.Results After treatment,the levels of TG and LDL-C in the two groups were significantly lower than those before treatment(P<0.05),and the observation group was significantly lower than the control group(P<0.05);the resting heart rate of the two groups after treatment was significantly slower than that before treatment(P<0.05),and the observation group was significantly slower than the control group(P<0.05);after treatment,the heart rate difference,FS,LVEF,and E/A of the two groups were significantly increased(P<0.05),and the observation group was significantly higher than the control group(P<0.05);the total effective rate of treatment in the observation group was 92.9%(39/42),the control
