摘要
目的研究芪参益气滴丸对慢性肺源性心脏病患者细胞因子的影响。方法选择慢性肺源性心脏病患者164例,按照数字法分为治疗组与对照组,每组82例。对照组给予氧疗、原发病治疗、强心、利尿等;治疗组在此基础上加用芪参益气滴丸口服,0.5 g/次,3次/d,连续应用28 d。观察2组患者的6 min步行距离(6 minutes walking test,6-MWT);治疗前后左心室射血分数(left ventricular ejection fractions,LVEF);测定治疗前后血清中白细胞介素4(Interleukin-4,IL-4)、白细胞介素6(Interleukin-6,IL-6)、白细胞介素12(Interleukin-12,IL-12)、干扰素-γ(Interferon-γ,IFN-γ)的含量以及氨基末端B型钠尿肽原(N-terminal pro bran natriuretic peptide,NT-proBNP)水平。结果治疗前,2组患者NT-proBNP、6-MWT、LVEF比较差异无统计学意义(P>0.05)。治疗28 d后,2组患者NT-proBNP浓度降低,6-MWT、LVEF升高,治疗组和对照组比较改善显著,差异有统计学意义(P<0.05);治疗前,治疗组与对照组患者IL-4、IL-6、IFN-γ、IL-12含量比较差异无统计学意义(P>0.05)。治疗后,2组IL-4、IL-6含量降低,IFN-γ、IL-12含量升高,治疗组较对照组变化明显,2组比较差异有统计学意义(P<0.05)。治疗前,慢性肺源性心脏病患者血清NT-proBNP与IFN-γ/IL-4比值呈负相关关系(r=-0.892,P=0.001)。即慢性肺源性心脏病患者心衰越重,NT-proBNP越高,二者比值越小。结论芪参益气滴丸能调整细胞因子水平,调节细胞应答平衡,改善心脏功能,缓解临床症状。
Objective To investigate the effects of Qishenyiqi dropping pill on cytokines in patients with chronic pulmonary heart disease.Methods A total of 164 patients with chronic pulmonary heart disease who were treated in our hospital from August 2016 to October 2018 were divided into observation group(n=82)and control group(n=82).The patients in control group were treated by oxygen therapy,primary disease treatment,cardiotonic and diuretic drugs,however,the patients in observation group,on the basis of control group,were treated by oral Qishenyiqi dropping pill,0.5g once,three times a day,for 28 days.The 6 minute walking test distance(6 MWT)and the left ventricular ejection fraction(LVEF)before and after treatment were measured and compared between the two groups.The levels of IL-4,IL-6,IL-12,IFN-γand NT proBNP were measured before and after treatment.Results Before treatment,there were no significant differences in NT proBNP,6 MWT,LVEF between the two groups(P>0.05).After 28-day treatment,the levels of NT proBNP were decreased significantly in both groups,however the 6 MWT,LVEF were significantly increased in both groups(P<0.05),moreover the improvement degree in observation group was more significant than that in control group(P<0.05).Before treatment there were no significantly differences in the levels of IL-4,IL-6 and IFN-γ,IL-12 between the two groups(P>0.05).After treatment,the levels of IL-4 and IL-6 were significantly decreased in both groups,however the levels of IL-12 were significantly increased in both groups,moreover,the changes in observation group were more significant than those in control group(P<0.05).Before treatment,there was a negative correlation between serum NT proBNP and the ratio of IFN-γ/IL-4 in patients with chronic pulmonary heart disease(P<0.01),that is,the more severe heart failure in patients with chronic pulmonary heart disease,the higher the NT proBNP and the smaller the ratio of IFN-γ/IL-4.Conclusion Qishenyiqi dropping pill can regulate the cytokine levels,regulate cell respons
