2型糖尿病周围神经病变患者缺氧诱导因子-1α及血管内皮生长因子的研究 被引量:13

Investigation of hypoxia-inducible factor-1αand vascular endothelial growth factor in type 2 diabetes with peripheral neuropathy

摘要 目的探讨2型糖尿病(type 2 diabetics,T2DM)周围神经病变患者血清缺氧诱导因子-1α(hypoxia-inducible factor-1α,HIF-1α)和血管内皮生长因子(vascular endothelial growth factor,VEGF)的变化及意义。方法选取2017年10月至2018年10月昆山市第一人民医院T2DM患者108例,根据是否合并糖尿病周围神经病变(diabetes with peripheral neuropathy,DPN),将患者分为T2DM组(53例)和DPN组(55例),并设正常对照组(NC组,50例),采用酶联免疫吸附法(enzyme linked immunosorbent assay,ELISA)测定所有患者血清HIF-1α、VEGF,记录3组的年龄、病程,检测相关生化指标,并进行比较分析。结果T2DM组血清HIF-1α、VEGF浓度分别为(486.23±136.01)pg/ml、(36.12±19.34)pg/ml,DPN组分别为(635.26±180.56)pg/ml、(51.18±24.72)pg/ml,明显高于NC组的(165.85±61.30)pg/ml、(10.92±2.64)pg/ml,差异均有统计学意义(P<0.05);DPN组的血清HIF-1α、VEGF浓度较T2DM组明显升高(P<0.05)。血清HIF-1α水平与TG、空腹血糖(fasting plasma glucose,FPG)、糖化血红蛋白(glycosylated hemoglobin,HbA1c)、VEGF呈正相关(r值分别为0.354、0.512、0.546、0.502),与HDL-C呈负相关(r=-0.511)。多元逐步回归分析显示,糖尿病病程、HbA1c、HIF-1α及VEGF是DPN的影响因素。结论T2DM患者HIF-1α及VEGF水平的变化在DPN的发生发展过程中可能起重要作用。 Objective To explore the changes and significance of hypoxia-inducible factor-1α(HIF-1α)and vascular endothelial growth factor(VEGF)in type 2 diabetics(T2DM)with peripheral neuropathy.Methods One hundred and eight patients with T2DM enrolled in our hospital from October 2017 to October 2018 were divided into two groups:the DM group(T2DM,n=53)and diabetic with peripheral neuropathy group(DPN,n=55),depending on whether the merger of DPN.Fifty healthy subjects were taken as control group(NC,n=50).Enzyme-linked immunosorbent assay(ELISA)was used to determinate the level of serum HIF-1αand VEGF.The age and duration of disease,biochemical indicators of the three groups were tested and recorded.Results The serum levels of HIF-1αand VEGF in group T2DM were(486.23±136.01)(36.12±19.34)pg/ml,and were(635.26±180.56)(51.18±24.72)pg/ml in group DPN.Both of them were significantly higher than those of group NC[(165.85±61.30),(10.92±2.64)pg/ml,P<0.05].The serum levels of HIF-1αand VEGF in group DPN were higher than those in group T2DM(P<0.05).The serum HIF-1αlevels were positively correlated with TG,fasting plasma glucose(FPG),glycosylated hemoglobin A1c(HbA1c),VEGF(r=0.354,0.512,0.546,0.502),and negatively correlated with HDL-C(r=-0.511).Multiple linear regression analysis showed the duration of diabetes,the level of HbA1c,HIF-1αand VEGF were the main factors of DPN(P<0.01).Conclusions The changes of HIF-1αand VEGF play an important role in occurrence and development of peripheral neuropathy in T2DM.
