摘要
目的探讨变应性鼻炎(allergic rhinitis,AR)患者外周血IL-33的表达水平。方法回顾性选择就诊的85例AR患者作为AR组,其中分为治疗前组和治疗后组;体检中心的健康体检者55例设为对照组。采用ELISA法检测所有研究对象外周血中IL-33的表达水平;通过鼻声反射仪对治疗前后的85例AR组患者进行鼻声反射测试并记录激发后鼻腔容积(NV)、鼻腔最小横截面积(MCA)、最小横截面距离前鼻孔距离(DCAN)和鼻阻力(NR),运用Pearson相关分析探讨AR组患者外周血IL-33的表达水平与上述指标的相关性。血清IL-33诊断中-重度AR的临床效能采用受试者操作特征(receiver operating characteristic,ROC)曲线分析。结果AR组患者外周血IL-33的表达水平显著高于对照组,差异有统计学意义(均P<0.05);治疗后AR组患者外周血IL-33表达水平显著低于治疗前,但仍高于对照组,差异均有统计学意义(均P<0.05)。相关性分析表明,AR组患者治疗前IL-33表达水平同NV、MCA、DCAN呈现显著负相关,相关系数分别为-0.546、-0.732和-0.418,同NR呈现显著正相关,相关系数为0.878;AR组患者治疗后IL-33表达水平同NV、MCA、DCAN和NR均无相关性。血清IL-33评估中-重度AR的ROC曲线下面积(AUC)为0.913(95%CI:0.868~0.956,P<0.05),当截断值为286.14 ng/mL时,此时特异度和敏感度分别为90.15%和92.56%。结论AR患者外周血IL-33表达水平显著升高,且可在一定程度上反映患者的病情严重程度。
Objective To investigate the serum IL-33 level in patients with allergic rhinitis.Methods A total of 85 patients with allergic rhinitis were selected and defined as Group AR.The Group AR group was divided into two sub-group(before-and after-treatment group)according to medical treatment.Another 55 healthy persons in the physical examination center in our hospital were defined as Group Control.The serum IL-33 level of all subjects were detected by ELISA methods.Before and after treatment,the nasal volume(NV),minimum cross-sectional area(MCA),minimum cross-sectional distance(DCAN)and nasal resistance(NR)of all 85 patients with allergic rhinitis were recorded by naso acoustic reflector.Pearson analysis was used to investigate the correlation between the serum IL-33 level and the above indexes in patients with allergic rhinitis.The application of serum IL-33 level in the diagnosis of moderate to severe allergic rhinitis was analyzed by receiver operating characteristic(ROC)curve.Results The serum levels of IL-33 in patients of Group AR was significantly higher than the Group Control(P<0.05);and the serum IL-33 levels in after-treatment group was significantly lower than that in before-treatment group,but still higher than that in Group Control(P<0.05).Before treatment,the serum IL-33 level was negatively correlated with NV,MCA and DCAN(Correlation coefficients=-0.546,-0.732 and-0.418,respectively).However,the serum IL-33 level was positively correlated with NR(Correlation coefficient=0.878).The serum IL-33 level was not correlated with NV,MCA,DCAN and NR after treatment.The area under curve(AUC)of serum IL-33 in the diagnosis of moderate to severe allergic rhinitis was 0.913(95%CI:0.868-0.956,P<0.05).When the cut-off value was 286.14 ng/mL,the specificity and sensitivity were 90.15%and 92.56%,respectively.Conclusion The serum IL-33 in patients with allergic rhinitis is significantly higher,reflecting the severity of the disease.
