摘要
目的研究舒筋活络膏对寒湿痹阻型膝骨性关节炎(KOA)患者关节液中白细胞介素IL-6和肿瘤坏死因子TNF-α含量的调节与干预作用。方法选取2015年12月—2016年12月在江苏省苏州市吴江区中医医院(苏州市吴江区第二人民医院)骨伤科门诊及住院治疗的寒湿痹阻型膝骨性关节炎患者42例,采用简单随机分组,将其分为治疗组(n=21)及对照组(n=21)。治疗组采用舒筋活络膏外敷,对照组采用复方南星止痛膏外敷,比较治疗前、后两组患者的膝关节液中炎性因子IL-6和TNF-α水平的变化。结果与治疗前相比,两组患者的IL-6含量显著降低,其中治疗组患者的IL-6降至(552.42±196.01)pg/mL,较治疗后对照组患者的(1656.97±252.37)pg/mL明显要低,差异有统计学意义(P<0.05);而治疗组和对照组患者治疗后的TNF-α水平分别为(9.21±5.02)pg/mL、(8.03±8.11)pg/mL,与治疗前相比改善不够明显,且治疗后两组间比较也差异无统计学意义(P>0.05)。结论舒筋活络膏治疗寒湿痹阻型膝关节骨性关节炎的疗效显著,可以明显降低膝关节液中炎性因子IL-6的水平,减少患者的疼痛,具有临床推广价值。
Objective To study the regulation and intervention effect of Shujin Huoluo ointment on interleukin-6(IL-6)and tumor necrosis factor-alpha(TNF-alpha)in joint fluid of patients with cold-dampness obstruction knee osteoarthritis(KOA).Methods 42 patients with knee osteoarthritis of cold-dampness obstruction type treated in orthopaedics and traumatology department of Wujiang district hospital of Jiangsu province from December 2015 to December 2016 were randomly divided into treatment group(n=21)and control group(n=21).The treatment group was treated with Shujin Huoluo ointment and the control group was treated with compound Nanxing Zhitong ointment.The changes of inflammatory factors IL-6 and TNF-alpha in knee joint fluid of the two groups before and after treatment were compared.Results Compared with that before treatment,the level of IL-6 in the two groups decreased significantly.The level of IL-6 in the treatment group decreased to(552.42±196.01)pg/mL,which was significantly lower than that in the control group(1656.97±252.37)pg/mL after treatment,the difference was statistically significant(P<0.05);the levels of TNF-αin the treatment group and the control group were(9.21±5.02)pg/ml and(8.03±8.11)pg/mL respectively,which were not significantly improved compared with those before treatment,and there was no statistically significant difference between the two groups after treatment(P>0.05).Conclusion Shujin Huoluo ointment is effective in treating knee osteoarthritis of cold-dampness obstruction type.It can significantly reduce the level of inflammatory factor IL-6 in knee joint fluid and reduce the pain of patients.It has clinical popularization value.
