摘要
目的探讨甲状腺瘤行低位小切口切除术的临床效果及安全性。方法选择该院2017年8月—2018年12月期间收治的甲状腺瘤手术患者65例,根据治疗方式不同分组,即对照组(n=30)和观察组(n=35),对照组采取传统手术治疗,观察组采取低位小切口切除手术治疗。比较两组患者手术相关指标、术后疼痛评分、美容程度及并发症情况。结果观察组患者术中出血量、手术切口及住院时间分别为(54.23±3.12)mL、(4.87±1.11)cm及(4.76±1.08)d,低于对照组的(80.43±3.89)mL、(7.32±1.34)cm及(7.23±1.66)d,差异有统计学意义(t=30.125、8.063、7.206,P<0.05)。观察组术后疼痛评分为(2.23±0.98)分,低于对照组(4.34±1.09)分,差异有统计学意义(t=8.217,P<0.05)。观察组美容效果评分为(8.54±1.34)分,对照组美容效果评分为(2.65±1.08)分,组间差异有统计学意义(t=19.619,P=0.000)。观察组术后并发症发生率为2.86%,低于对照组并发症发生率26.67%,差异有统计学意义(χ^2=5.811,P=0.006)。结论与传统手术比较,低位小切口切除术治疗甲状腺瘤疗效更为显著,创伤更小,术后疼痛程度较低,且不良反应少,有利于患者术后康复,该术具有较高的应用价值。
Objective To investigate the clinical effect and safety of low incision thyroidectomy for thyroid tumors.Methods 65 patients with thyroma who underwent thyroidectomy in our hospital from August 2017 to December 2018 were divided into control group(n=30)and observation group(n=35)according to different treatment methods.The control group was treated with traditional surgery,while the observation group was treated with low incision resection.The operation related indexes,pain score,cosmetic degree and complications were compared between the two groups.Results The intraoperative blood loss,surgical incision,and length of stay in the observation group were(54.23±3.12)mL,(4.87±1.11)cm,and(4.76±1.08)d,which were lower than those in the control group(80.43±3.89)mL and(7.32±1.34)cm and(7.23±1.66)d,the difference was statistically significant(t=30.125,8.063,7.206,P<0.05).The postoperative pain score of the observation group was(2.23±0.98)points,which was lower than that of the control group(4.34±1.09)points.The difference was statistically significant(t=8.217,P<0.05).The cosmetic effect score in the observation group was(8.54±1.34)points,and the cosmetic effect score in the control group was(2.65±1.08)points.The difference between the groups was statistically significant(t=19.619,P=0.000).The incidence of postoperative complications in the observation group was 2.86%,which was lower than that in the control group by 26.67%.The difference was statistically significant(χ^2=5.811,P=0.006).Conclusion Compared with traditional surgery,low incision thyroidectomy is more effective,less traumatic,less painful and less adverse reactions.It is conducive to the recovery of patients after operation.It has high application value.
