摘要
目的评估日常临床工作状态下全麻患者在麻醉恢复室(PACU)期间术后肌松残留(PORC)的发生率。方法该研究选择在2011年6月—2018年6月武隆区人民医院收治的542例全麻患者,遵循前瞻性、观察性队列研究设计,分别采用TOFR<0.7和TOFR<0.9的标准判断是否存在PORC,观察日常临床工作状态下进入和离开PACU时PORC的发生率。结果采用TOFR<0.7标准则PORC发生率为12.9%(70/542),采用TOFR<0.9标准则PORC发生率为30.6%,多因素Logistic回归分析指出,年龄超过65岁(OR=1.301)、进入PACU体温低于36℃(OR=1.872)、体重指数高(OR=1.231)是全身麻醉患者进入PACU时出现PORC(TOFR<0.9)的独立危险因素,而使用肌松拮抗剂(OR=0.435)、延长末次使用肌松药至进入PACU间隔时间(OR=0.226)是减少PORC发生的保护性因素。结论在日常临床工作状态下,患者在全身麻醉术后进入PACU时PORC有较高发生率。
Objective To evaluate the incidence of postoperative muscle relaxant(PORC)in general anesthesia patients during routine anesthesia in the anesthesia recovery unit(PACU).Methods This study selected 542 patients under general anesthesia admitted to the People's Hospital of Wulong district from June 2011 to June 2018.They were designed according to a prospective and observational cohort study.The criteria of TOFR<0.7 and TOFR<0.9 were used to determine whether The presence of PORC,observe the incidence of PORC when entering and leaving PACU under daily clinical working conditions.Results The incidence of PORC was 12.9%(70/542)with TOFR<0.7,and the incidence of PORC was 30.6%with TOFR<0.9.Multivariate logistic regression analysis indicated that the age was over 65 years(OR=1.301),entering the PACU with a body temperature below 36°C(OR=1.872)and a high body mass index(OR=1.231)are independent risk factors for PORC(TOFR<0.9)in general anesthesia patients when entering the PACU.Muscle relaxants(OR=0.435).Extending the interval between the last use of muscle relaxants and entering the PACU(OR=0.226)was a protective factor that reduces the occurrence of PORC.Conclusion Under normal clinical working conditions,patients have a higher incidence of PORC when they enter PACU after general anesthesia.
