改良Kwak甲状腺影像报告和数据系统在甲状腺结节诊断中的应用价值 被引量:1

摘要 目的:探讨改良Kwak甲状腺影像报告和数据系统(thyroid imaging reporting and data system,TI-RADS)在甲状腺结节诊断中的应用价值。方法:回顾性分析2018年1-12月本院甲状腺结节患者150例(160个结节)的临床资料。对所有患者实行改良Kwak TI-RADS分级,以术后病理结果为金标准,分析改良Kwak TI-RADS在甲状腺良恶性结节鉴别诊断中的价值。结果:良性结节的实性结节、低回声、纵横比>1、边界不规则、有钙化比例均明显低于恶性结节(P<0.05)。良性结节改良Kwak TI-RADS分级为2、3级的例数均高于恶性结节,而4b、4c及5级的例数均低于恶性结节(P<0.05)。改良Kwak TI-RADS分级临界值为3.50,改良Kwak TI-RADS分级诊断良恶性甲状腺结节的敏感度为0.821、特异度为0.803、准确度为0.806。结论:改良Kwak TI-RADS分级对甲状腺结节良恶性有较强的鉴别作用,临床上可以借鉴。 Objective: To explore the application value of modified Kwak thyroid imaging reporting and data system(TI-RADS) in thyroid nodule diagnosis. Method: The clinical data of 150 patients(160 nodules) with thyroid nodules in our hospital from January to December 2018 were analyzed retrospectively. All patients were graded with modified Kwak TI-RADS, and the value of modified Kwak TI-RADS in differential diagnosis of benign and malignant thyroid nodules was analyzed by postoperative pathological results as the gold standard. Result: The solid nodule, hypoechoic nodule, aspect ratio>1, irregular border and calcification ratio of benign nodule were significantly lower than those of malignant nodule(P<0.05). The number of cases with grade 2 and 3 of modified Kwak TI-RADS in benign nodules were higher than those of malignant nodules, while the number of cases with grade 4b, 4c and 5 were lower than that of malignant nodules(P<0.05). The critical value of modified Kwak TI-RADS score was 3.50. The sensitivity of modified Kwak TI-RADS grade to diagnose benign and malignant thyroid nodules was 0.821, the specificity was 0.803 and the accuracy was 0.806. Conclusion: Modified Kwak TI-RADS grade has a strong differentiating effect on benign and malignant thyroid nodules, which can be used for reference in clinic.
