摘要
目的探讨Pentacam眼前节分析系统观察配戴硬性角膜接触镜(RGPCL)对圆锥角膜的治疗效果。方法通过回顾性分析,将38例(72只眼)在兰州大学第一医院就诊的患者根据Pentacam三维眼前节分析系统检查的角膜形态分为轻中度圆锥角膜组和重度圆锥角膜组。观察两组患者配戴RGPCL前、配戴后1、6、12和24月时的最佳矫正视力(BCVA),以及配戴RGPCL24月角膜曲率、角膜厚度及角膜前后表面高度的变化。结果轻中度圆锥角膜组和重度圆锥角膜组配戴RGPCL治疗后1、6、12和24月BCVA与戴镜前BCVA比较,差异均具有统计学意义(P<0.05);轻中度圆锥角膜组和重度圆锥角膜组RGPCL治疗前BCVA差异有统计学意义(P<0.05);RGPCL治疗后1、6、12、24月两组间BCVA比较差异无统计学意义(P>0.05)。Pentacam眼前节分析系统观察角膜相关参数变化,角膜前表面最大曲率值治疗后24月与治疗前比较有所下降,差异有统计学意义(P<0.05),角膜最薄点厚度、前后表面高度治疗后24月与治疗前比较差异无统计学意义(P>0.05)。结论RGPCL对圆锥角膜患者是安全有效的,Pentacam眼前节分析系统能够准确检测圆锥角膜患者角膜形态的变化。
Objective To investigate the therapeutic effect of rigid corneal contact lens(RGPCL)on keratoconus by Pentacam anterior segment analysis system.Methods In this retrospective analysis,38 cases(72 eyes)in The First Hospital of Lanzhou University were divided into mild to moderate keratoconus group and severe keratoconus group according to the corneal shape through Pentacam anterior segment analysis system.The best corrected visual acuity of two groups were observed before and 1 month,6,12and 24 months after wearing RGPCL.The changes of corneal curvature,corneal thickness,anterior corneal surface height and posterior corneal surface height of patients with keratoconus were also observed before and 24 months after RGPCL wearing.Results After 1 month,6,12and 24months wearing RGPCL,the changes of BCVA were statistically significant in mild to moderate keratoconus group and severe keratoconus group,compared with before treatment(P<0.05).There was no significant difference in BCVA between mild to moderate keratoconus group and severe keratoconus group in 1 month,6,12 and 24 months after treatment(P>0.05).By using Pentacam anterior segment analysis system to observe the changes of corneal parameters in mild to moderate keratoconus group and severe keratoconus group,the maximum curvature of the anterior surface of cornea decreased,the difference was statistically significant after 24 months of treatment(P<0.05).There was no significant difference in the thinnest corneal thickness,the height of the anterior surface and the height of the posterior surface of the cornea,before and 24 months after wearing RGPCL(P>0.05).Conclusions Wearing RGPCL is safe and effective for keratoconus,and the Pentacam anterior segment analysis system can accurately detect the changes of corneal morphology in keratoconus patients.
