摘要
目的:对比腹腔镜下中间入路与尾侧联合内侧入路在右半结肠癌根治性切除术中的应用价值。方法:采用等距随机抽样法选取广安市人民医院2017年1月—2018年6月收治的102例结肠癌患者,分为采取尾内侧入路的观察组及采取中间入路的对照组各51例,两组术式均为腹腔镜下右半结肠癌根治性切除术,对手术治疗效果进行对比。结果:观察组手术时间、术中出血量、排气时间较对照组更短/少,差异有统计学意义(P<0.05),排便时间、镇痛时间、引流管拔除时间、引流量、进食时间、住院时间与对照组相当,差异无统计学意义(P>0.05);两组术前肿瘤标志物差异不明显,术后观察组均明显低于同期对照组,差异有统计学意义(P<0.05);术后统计并比较两组并发症发生率发现差异无统计学意义(P>0.05)。结论:在腹腔镜下右半结肠癌根治性切除术中尾侧联合内侧入路取得的临床治疗效果较中间入路更具优势,可作为优选手术入路加以推广使用。
Objective:To compare the value of laparoscopic intermediate approach and caudal side combined with medial approach in the radical resection of right colon cancer.Methods:102 patients with colon cancer who were enrolled in our hospital from January,2017 to June,2018 were divided into observation group with tail medial approach and control group with intermediate approach,using an isometric random sampling method.The group was treated with laparoscopic radical resection of right colon cancer,and the effects of surgery were compared.Results:The operation time,intraoperative blood loss and exhaust time of the observation group were shorter or smaller than the control group.The difference was statistically significant(P<0.05),defecation time,analgesia time,drainage tube removal time,drainage volume,The eating time and hospitalization time were comparable to the control group,and the difference was not statistically significant(P>0.05).There was no significant difference in preoperative tumor markers between the two groups.The postoperative observation group was significantly lower than the control group,and the difference was statistically significant(P<0.05);postoperative statistics and comparison of the incidence of complications between the two groups found no significant difference(P>0.05).Conclusion:The clinical effect of laparoscopic radical resection of right colon cancer in the radical resection of the caudal side of the colon is more advantageous than the intermediate approach.It can be used as a preferred surgical approach.
