甲状腺功能亢进性心脏病患者血浆B型钠尿肽的变化与临床意义研究 被引量:2

摘要 目的该次研究分析在甲状腺功能亢进性心脏病患者中血浆B型钠尿肽的变化与临床意义。方法样本从该院甲状腺功能亢进性心脏病患者中选取50例,并选取同期接受治疗的50例单纯甲状腺功能亢进患者作为对照研究,在两组患者初诊时对其血浆B型钠尿肽(BNP)、游离三碘甲腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺激素(TSH)等指标进行检测,对比患者治疗初期和治疗3个月后的心功能分级与血浆B型钠尿肽(BNP)的关系。结果对比两组患者确诊时的甲状腺功能水平对比差异无统计学意义(P>0.05),B型钠尿肽(BNP)水平差异有统计学意义(P<0.05);患者B型钠尿肽(BNP)水平随着心功能不全级别升高而随之提升;在治疗1周后,患者甲状腺功能水平下降情况与治疗前差异无统计学意义(P>0.05),B型钠尿肽(BNP)水平与治疗前差异有统计学意义(P<0.05);在治疗3个月后,两种指标均下降明显,且差异有统计学意义(P<0.05)。结论甲状腺功能亢进性心脏病患者血浆B型钠尿肽水平可作为鉴别诊断指标,同时还能作为临床治疗效果的监测指标。 【Objective】To analyzed the changes and clinical significance of plasma B-type natriuretic peptide in patients with hyperthyroid heart disease.【Methods】A total of 50 patients with hyperthyroid heart disease were enrolled in the study.Fifty patients with hyperthyroidism who underwent concurrent treatment were selected as control subjects.Plasma B-type sodium was given to the two groups at the time of initial diagnosis.Plasma B-type natriuretic peptide(BNP),FT3,FT4,TSH and other indicators were tested to compare the relationship between cardiac function grading and BNP in the initial stage of treatment and after 3 months of treatment.【Results】There was no statistical difference in thyroid function between the two groups at diagnosis(P>0.05),and there was a statistical difference in BNP level(P<0.05).The higher the level of cardiac insufficiency,the higher the level of BNP in patients.After one week of treatment,there was no significant difference in the level of thyroid function before and after treatment(P>0.05),and the level of BNP was significantly different from that before treatment(P<0.05).After 3 months of treatment,both indicators decreased significantly with statistical differences(P<0.05).【Conclusion】The level of plasma B-type natriuretic peptide can be used as a differential diagnosis indicator for patients with hyperthyroid heart disease,as well as a monitoring indicator for clinical treatment.
