摘要
目的探讨加速康复外科(ERAS)在人工髋关节置换术(THA)围术期护理中的应用价值,为临床提供新方法、新理念。方法选取2017年1月~2019年1月我院收治的60例THA患者,按收治的时间顺序分为实验组和对照组,每组各30例。实验组应用ERAS理念围术期护理,对照组采取传统护理方法,比较两组的髋关节功能、满意度、营养状况、日常生活活动能力。结果实验组术后1、2、4周髋关节Harris评分均高于对照组,差异有统计学意义(P<0.05);实验组手术日、术后2 d、术后1周的营养评分高于对照组,差异均有统计学意义(P<0.05);实验组术后1、2、4周的日常生活活动能力(ADL)量表(Barthel)评分高于对照组,差异均有统计学意义(P<0.05);实验组自觉恢复情况、护理态度、技术操作、康复指导质量、其他评分及满意度总分高于对照组,差异均有统计学意义(P<0.05)。结论ERAS明显优于传统护理,不仅促进患者康复,提高生存质量,且对护理质量有明显的促进和提高作用,有实际应用价值。
Objective To investigate the application value of enhanced recovery after surgery(ERAS)in the perioperative nursing care of artificial hip replacement(THA),and provide new methods and new ideas or clinic.Methods A total of 60 patients with THA treated in our hospital from January 2017 to January 2019 were selected and divided into experimental group and control group according to the time sequence,30 cases in each group.The experimental group received perioperative nursing with the concept of ERAS,while the control group received traditional nursing methods.The hip function,satisfaction,nutritio status,daily living ability were compared between two groups.Results The Harris scores of hip joints in the experimental group at 1,2,and 4 weeks after surgery were all higher than those in the control group,with statistical differences(P<0.05);the nutritional scores of the experimental group on the day of operation,2 days and one week after operation were all higher than those of the control group,with statistical differences(P<0.05);the scores of activities of daily life function(ADL)scale(Barthel)scores in the experimental group were higher than those in the control group 1,2 and 4 weeks after operation,and the differences were statistically significant(P<0.05);the total scores of conscious recovery,nursing attitude,technical operation,quality of rehabilitation guidance,other scores and satisfaction of the experimental group were higher than those of the control group,with statistical differences(P<0.05).Conclusion ERAS is obviously better than the traditional nursing.It not only promotes the rehabilitation of patients and improves the quality of life,but also significantly promotes and improves the quality of nursing.It has practical application value.
