摘要
目的:探讨极外侧椎间融合术(extreme lateral interboy fusion,XLIF)治疗腰椎退行性疾患的应用价值。方法:回顾性分析40例应用单节段XLIF治疗的腰椎退行性疾病的病例资料,观察视觉模拟疼痛评分(visual analogue scale,VAS)和Oswestry功能障碍指数(Oswestry disability index,ODI)的改善情况,摄腰椎片并评估内植物及椎间融合情况。结果:40例患者中,手术时间为35~65min,平均45.5±16.7min;术中出血量30~100m L,平均63.7±35.2m L;术后住院日3~7天,平均4.5±2.1天;所有患者术后VAS和ODI评分均显著降低,并发症发生率为20.0%,且在随访过程中均得到不同程度缓解或消失。结论:XLIF作为一种微创脊柱融合技术,可获得良好的短期疗效,具有临床应用价值,但其中远期的随访结果有待进一步观察。
Objective:To investigate the application value of extreme lateral interboy fusion(XLIF)for lumbar degenerative diseases.Methods:A ret rospective analysis was carried out on data of 40 cases of lumbar degenerative diseases who accepted single segment XLIF to observe their improve ment of visual analogue scale(VAS)and Oswestry disability index(ODI),and evaluated the implant and intervertebral fusion by lumbar X-ray film Results:In the 40 cases of patients,operation time was 35~65(45.5±16.7)min;intraoperative blood loss was 30~100(63.7±35.2)m L;hospitaliza tion time after surgery was 3~7(4.5±2.1)days;VAS and ODI scores of all patients after surgery were significantly decreased,incidence of complica tions was 20.0%and had been released or disappeared during follow-up.Conclusion:As a kind of minimally invasive spinal fusion technique,XLIF can achieve a good short-term efficacy,which makes it deserve clinical application,but its long-term follow-up results need to be further observed.
