摘要
目的探讨外耳再造术后冻伤的治疗方法。方法通过病史和体格检查明确再造耳冻伤的诊断。及时给予复温,改善微循环、消肿、抗渗出、预防感染等治疗。应用5%葡萄糖溶液200 ml,维生素C 1.0 g,地塞米松15 mg每日1次静滴;0.9%氯化钠溶液100 ml,头孢唑林钠0.5 g每日2次静滴;低分子右旋糖酐250 ml每日一次静滴;5%葡萄糖溶液200 ml,丹参注射液10 ml每日一次静滴;云南白药0.5 g每日3次口服;红光(600~700 nm)照射治疗20分钟,照射距离30 cm,功率设置3 W,每日2次。结果3日后肿胀逐渐消退,再造耳各部分轮廓基本显现,肤色恢复至接近正常。结论本案例采用的治疗方法,适用于外耳再造术后冻伤部分病例的治疗。
Objective To explore the treatment method of frostbite of reconstructed ear after operation.Methods Patients who were diagnosed as frostbite reconstructed ear by history and physical examination were included in this study. Measures such as recover temperature,improve microcirculation,reduce swelling,anti-exudate,anti-infection were applied to these patients immediately being in hospital. The specific treatment measures were clarified as follows.200 ml of 5% glucose solution combined with 1. 0 g of vitamin C and 15 mg dexamethasone were used one time daily by intravenous drip.100 ml of 0.9% sodium chloride injection with 0.5 g of cephazolin sodium were used two times daily by intravenous drip.250 ml of low Molecular Dextran were used one time daily by intravenous drip.200 ml of 5% glucose solution with 10 ml of Danshen injection were used one time daily by intravenous drip.0.5 g of Yunnan Baiyao capsule was taken orally three times daily.In addition,we applied red light machine with a length of 600 to 700 nm to shine the frostbite areas for 20 minutes two times a day,keeping a distance of 30 cm,and setting the power at 3 W. Results After three days of treatment,swelling was gradually faded away,and the color of the skin on the surface of the frostbite ear was return to normal.The profile of each structure of the reconstructed ear appeared again after treatment.Conclusions The measures we adopted in this study are effective in the treatment of frostbite of reconstructed ear.
