摘要
目的探讨彩色多普勒超声对眼缺血综合征(ocular ischemia syndrome,OIS)患者行颈动脉支架植入术疗效的评价及临床应用价值。方法OIS患者72例(83眼)均行颈动脉支架植入术,根据颈动脉狭窄程度和最佳矫正视力评价临床疗效,并根据疗效分为治疗有效组52眼和治疗无效组31眼,采用经颈部和眼部彩色多普勒超声检查测定2组手术前、后颈内动脉、眼动脉和视网膜中央动脉收缩期峰值血流速度(peak systolic velocity,PSV)、舒张末期峰值血流速度(end diastolic velocity,EDV),计算阻力指数(resistance index,RI)。结果83眼术后颈内动脉、眼动脉及视网膜中央动脉PSV[(43.01±7.06)、(23.51±6.45)、(12.86±2.34)cm/s]、EDV[(13.69±5.14)、(11.71±5.16)、(6.86±0.34)cm/s]均明显高于术前[PSV(28.76±5.42)、(12.86±3.42)、(8.41±3.12)cm/s,EDV(8.26±4.08)、(5.16±2.14)、(3.26±1.52)cm/s](P<0.05),RI(0.68±0.05、0.50±0.11、0.47±0.08)低于手术前(0.71±0.08、0.60±0.14、0.51±0.09)(P<0.05);治疗有效组颈内动脉、眼动脉及视网膜中央动脉PSV[(49.25±8.06)、(31.65±7.16)、(17.02±3.21)cm/s]、EDV[(14.02±5.23)、(16.02±4.02)、(8.96±0.69)cm/s]高于治疗无效组[PSV(32.54±7.42)、(9.95±2.58)、(6.88±1.69)cm/s,EDV(5.10±2.18)、(4.48±1.52)、(3.34±1.21)cm/s](P<0.05),RI(0.72±0.11、0.49±0.08、0.47±0.09)低于治疗无效组(0.84±0.25、0.55±0.13、0.51±0.04)(P<0.05)。结论颈动脉支架植入术有利于改善OIS患者血管血流动力学参数,彩色多普勒超声的血管血流动力学参数对手术疗效有重要评价作用。
Objective To investigate the clinical value of color Doppler ultrasound to evaluating the efficacy of carotid stent implantation on ocular ischemia syndrome(OIS).Methods Seventy-two patients(83 eyes)with OIS were performed carotid artery stent implantation.The clinical efficacy was evaluated according to carotid artery stenosis and best corrected visual acuity(BCVA).The patients were divided into effective treatment group(52 eyes)and ineffective treatment group(31 eyes).The peak systolic velocity(PSV)and end diastolic velocity(EDV)of internal carotid artery,ophthalmic artery and central retinal artery were detected by transcervical and eye color Doppler ultrasound before and after operation.The resistance index(RI)was calculated.Results The PSV values of internal carotid artery,ophthalmic artery and central retinal artery((43.01±7.06),(23.51±6.45),(12.86±2.34)cm/s)and EDV values((13.69±5.14),(11.71±5.16),(6.86±0.34)cm/s)were higher,while the RI values(0.68±0.05,0.50±0.11,0.47±0.08)were lower after operation than those before operation(PSV:(28.76±5.42),(12.86±3.42),(8.41±3.12)cm/s;EDV:(8.26±4.08),(5.16±2.14),(3.26±1.52)cm/s;RI:0.71±0.08,0.60±0.14,0.51±0.09)(P<0.05).Effective treatment group had higher PSV values((49.25±8.06),(31.65±7.16),(17.02±3.21)cm/s)and EDV values((14.02±5.23),(16.02±4.02),(8.96±0.69)cm/s)but lower RI values(0.72±0.11,0.49±0.08,0.47±0.09)of internal carotid artery,ophthalmic artery and central retinal artery than ineffective treatment group PSV:(32.54±7.42),(9.95±2.58),(6.88±1.69)cm/s;EDV:(5.10±2.18),(4.48±1.52),(3.34±1.21)cm/s);RI:0.84±0.25,0.55±0.13,0.51±0.04)(P<0.05).Conclusion The hemodynamic parameters of blood vessels could be improved obviously after carotid stent implantation,and color Doppler ultrasound is of great importance in evaluating the operative efficacy in OIS patients.
