3.0T磁共振弥散加权成像(DWI)在食管癌放疗疗效评价中的应用价值 被引量:4

摘要 目的:探讨3.0T磁共振弥散加权成像在食管癌患者放疗疗效评价中的应用价值。方法:选取我院2015年1月-2019年3月间收治的食管癌患者22例作为研究对象,所有患者均接受三维适形放疗或调强放疗,分别用食管镜、X线钡餐、CT和磁共振测量病灶长度,采用磁共振DWI测量放疗前后ADC的变化。结果:四种方法测量病灶长度结果具有统计学差异(F=6.947,P<0.05),两两比较发现CT测量结果最高,且与其他三种方法比较均均有统计学差异(q=8.423,4.664,5.871,P<0.05),其他两两比较无统计学差异。但从平均测量数值来看,磁共振测量结果与X线钡餐最接近。采用CT测量患者病灶平均最大为(3.78±0.90)cm,而磁共振测量结果为(3.80±0.94),两者无统计学差异(t=1.266,P>0.05)。当取不同b值时,放疗后的ADC值均低于放疗前,差异显著(t=8.547,8.661,6.529,P<0.05),这表明放疗后病变明显减轻。结论:磁共振DWI在评价食管癌放疗疗效时能够极好的弥补钡餐造影和CT的不足,通过比较放疗前后病灶区域ADC值能够较为准确的对疗效进行定量评估,进而指导临床预后。 Objective:To explore the value of 3.0 T MR diffusion weighted imaging in evaluating the radiotherapy efficacy of esophageal cancer patients.Methods:Twenty-two patients with esophageal cancer admitted to our hospital from January 2015 to March 2019 were selected as the study subjects.All patients received three-dimensional conformal radiotherapy or intensity-modulated radiotherapy.The lesion length was measured by esophagoscopy,X-ray barium meal,CT and magnetic resonance,and ADC was measured by magnetic resonance DWI before and after radiotherapy.Results:The results of four methods for measuring the length of lesions were statistically different(F=6.947,P<0.05).Comparing the two methods,the results of CT measurement were the highest,and there were significant differences compared with the other three methods(q=8.423,4.664,5.871,P<0.05).There was no statistical difference between the other two methods.However,from the average measurement value,the results of magnetic resonance measurement are closest to those of X-ray barium meal.The average maximum size of lesions measured by CT was(3.78+0.90)cm,while that measured by magnetic resonance was(3.80+0.94).There was no significant difference between the two methods(t=1.266,P>0.05).When different b values were taken,the ADC values after radiotherapy were significantly lower than those before radiotherapy(t=8.547,8.661,6.529,P<0.05),indicating that the pathological changes were significantly reduced after radiotherapy.Conclusion:MR DWI can make up for the deficiencies of barium meal radiography and CT in evaluating the curative effect of radiotherapy for esophageal cancer.By comparing the ADC values of the lesion area before and after radiotherapy,the curative effect can be evaluated quantitatively and the clinical prognosis can be guided.
