摘要
总结李思宁教授治疗心脏神经官能症的临证经验。李思宁教授认为心脏神经官能症可归属中医"郁证"范畴;情志失调是心脏神经官能症的主要致病因素,肝气不舒及心失所养为其病机关键,心肝是其主要病位;治疗心脏神经官能症的原则为心肝同调,虚实辨证,三因制宜,身心同治。治疗心脏神经官能症时,应在疏肝、柔肝、平肝的基础上,还需要重视宁心、养心、清心;心脏神经官能症的虚实辨证有性别差异,女性患者以虚证或虚实夹杂为主,男性患者以实证为主;心脏神经官能症可辨证为肝气郁结型、气郁化火型、心脾两虚型、心肾阴虚型,分别可采用柴胡疏肝散、丹栀逍遥散、归脾汤、天王补心丹加减治疗。
The experience of Professor LI Si-Ning in differentiating and treating cardiac neurosis was summarized.Professor LI Si-Ning believes that cardiac neurosis can be classified into the stagnation syndrome of traditional Chinese medicine, emotional disorder contributes to its primary pathogenic factor, disorder of liver qi and deficient cultivation of heart are the vital pathogenesis of cardiac neurosis,and the disease is mainly correlated with heart and liver. The therapeutic principles for the cardiac neurosis are to simultaneously regulate heart and liver,differentiate the deficiency and excess syndromes, treat the disease adapted to the climate,individuality,environment, and treat both of the physique and the spirit. The therapies for the cardiac neurosis should be focused on soothing liver,softening liver,and calming liver,and also should include the methods of calming heart, cultivating heart, and clearing heart. The deficiency and excess syndromes of cardiac neurosis are correlated with the gender,the female patients are predominated by deficiency syndrome or mixture of deficiency and excess syndrome, and the male patients are predominated by excess syndrome. Cardiac neurosis can be differentiated into the syndromes of liver qi stagnation,stagnated qi transforming into fire,deficiency of both heart and spleen,and deficiency of heart and kidney yin,which can be treated by the modification of Chaihu Shugan Powder,Danzhi Xiaoyao Powder,Guipi Decoction,Tianwang Buxin Pills respectively.
