摘要
目的对接受血液透析的肾衰竭患者进行低磷饮食干预,观察对营养状况、血磷、血糖管理及心理状况的影响。方法选择三门峡市中心医院2017年4月-2018年4月收治的80例肾衰竭血液透析患者进行研究,按照患者接受饮食干预的不同方式进行分组,观察组40例、对照组40例,对照组给予常规饮食干预,观察组给予低磷饮食干预,比较两组营养状况、心理状况、血磷、血糖管理情况。结果观察组干预后前清蛋白、清蛋白、血磷水平分别为(0.38±0.11)g/L、(35.21±2.46)g/L、(1.88±0.35)mmol/l,均优于对照组[(0.31±0.13)g/L、(31.42±3.16)g/L、(2.10±0.46)mmol/l,P<0.05];观察组低血糖发生率为2.50%,对照组发生率20.00%,组间对比差异有统计学意义(P<0.05);观察组干预后心理状况评分(4.26±1.19)分,对照组评分(6.02±1.35)分,组间对比差异有统计学意义(P<0.05)。结论肾衰竭血液透析病人接受低磷饮食干预能够更明显改善患者营养状况,减轻不良心理,改善血磷以及血糖水平,值得临床推广。
Objective To observe the effects of low-phosphorus diet intervention on nutritional status,blood phosphorus,blood glucose management and psychological status of renal failure patients receiving hemodialysis.Methods Selected Sanmenxia Central Hospital from April 2017 to April 2018 study of 80 cases of renal failure patients with hemodialysis,according to the patients received for the different ways of diet intervention group,observation group 40 cases,control group 40 cases,control group given conventional diet intervention,observation group was given a low phosphorus diet intervention,compared two groups of nutritional status,psychological status,blood phosphorus,blood sugar management.Results The levels of prealbumin,albumin and serum phosphorus in the observation group were(0.38±0.11)g/L,(35.21±2.46)g/L and(1.88±0.35)mmol/L,respectively,which were superior to those in the control group[(0.31±0.13)g/L,(31.42±3.16)g/L and(2.10±0.46)mmol/L].The incidence of hypoglycemia in the observation group was 2.50%,which was significantly lower than that of the control group(20.00%),and the difference between groups was statistically significant(P<0.05).After intervention,scores of psychological status in the observation group were(4.26±1.19)and those in the control group were(6.02±1.35),which were significantly better in the observation group than in the control group,and the difference was statistically significant(P<0.05).Conclusion The intervention of low-phosphorus diet in hemodialysis patients with renal failure can significantly improve the nutritional status of patients,reduce adverse psychology,and improve the level of blood phosphorus and blood glucose,which is worthy of clinical promotion.
