摘要
目的探讨基于3D人体扫描系统制作的支具用于青少年特发性脊柱侧弯(AIS)非手术治疗的治疗效果及生存质量。方法选取自北部战区总医院骨科脊柱病区2015年1月至2018年1月收治的42例AIS患者为研究对象,按取模方法分为传统支具组(n=20)及3D取模组(n=22)。传统支具组患者采用石膏取模制作支具,3D取模组采用3D扫描快速建模制作支具。观察两组患者的Cobb角及胸椎后凸角(TK)、腰椎前凸角(LL)、矢状位平衡(SVA)变化。末次随访时,比较两组患者的脊柱侧弯研究学会22项量表评分(SRS-22)、健康调查简表评分(SF-36)。结果随访1年及末次随访时,两组患者的Cobb角均小于初次随访,且3D取模组小于传统石膏取模组,差异均无统计学意义(P>0.05)。随访结束时,两组患者的TK、LL、SVA均较治疗前有一定程度好转,组间比较,差异无统计学意义(P>0.05)。SRS-22量表评分中,3D取模组的自我形象、心理健康及治疗满意度评分均明显高于传统支具组的患者,差异有统计学意义(P<0.05)。3D取模组的SF-36总评分明显高于支具组,差异有统计学意义(P<0.05)。结论基于3D人体扫描系统取模是一种安全、方便、快捷的支具取模方法,利用此方法制作的支具能有效控制并缓解AIS患者病情发展,提高生活质量,易于患者接受并能坚持穿戴,有利于长期的治疗。
Objective To investigate the therapeutic effect and quality of life of a brace made based on 3 D body scanning system for the non-surgical treatment of adolescent idiopathic scoliosis(AIS).Methods A retrospective study was performed on 42 cases of patients with AIS who were admitted from January 2015 to January 2018.According to the method of mold taking,patients were divided into the traditional brace group(n=20)and 3 D brace group(n=22).In the traditional brace group,plaster mold was taken to make the brace,while in the 3 D brace group,3 D scanning rapid modeling was used to make the brace.The changes of Cobb angle,thoracic kyphosis(TK)angle,lumbar lordosis(LL)angle and sagittal balance(SVA)were observed.At the last follow-up,the spine scoliosis study society 22-item scale score(SRS-22)and the MOS item short from health survey(SF-36)were compared between the two groups.Results At the 1-year follow-up and the last follow-up,Cobb angle of patients in both groups was smaller than that of patients in the first follow-up,and the 3 D brace group was smaller than the traditional brace group,with no statistically significant difference(P>0.05).At the end of the follow-up,TK,LL and SVA of the two groups were improved to a certain extent compared with those before treatment,and there was no statistically significant difference between the two groups(P>0.05).In the SRS-22 scale,the self-image,mental health and treatment satisfaction scores of the 3 D brace group were significantly higher than those of the traditional brace group,with statistically significant differences(P<0.05).The total SF-36 score of the 3 D brace group was significantly higher than that of the conventional brace group(P<0.05).Conclusion The 3 D body scanning system is a safe,convenient and fast method for formwork removal.The scaffold made by this method can effectively control and alleviate the development of AIS patients′condition,improve the quality of life,be easy for patients to accept and wear,and be conducive to long-term treatment.
