摘要
目的探讨拔牙窝内植入富血小板纤维蛋白(platelet rich fibrinogen,PRF)混合Bio⁃Oss®骨粉进行牙槽嵴保存术的效果,为临床应用提供证据。方法进行牙槽嵴保存术的患者36例,36个拔牙位点,分成两组:PRF混合Bio⁃Oss®骨粉组(试验组)和单独使用Bio⁃Oss®骨粉组(对照组),在术前和术后6个月拍摄CBCT测量牙槽骨高度和宽度的变化,术后1周临床评估早期软组织愈合和疼痛感觉情况。结果试验组骨高度变化(⁃1.48±0.40)mm和对照组(⁃1.49±0.23)mm差异无统计学意义。试验组骨宽度变化(⁃1.09±0.42)mm和对照组宽度变化(⁃1.35±0.22)mm差异有统计学意义(Z=⁃2.63,P=0.01)。试验组术后疼痛评分2.39±1.20,对照组3.39±1.65,差异有统计学意义(t=⁃2.083,P=0.045);试验组和对照组软组织愈合差异无统计学意义。结论在牙槽嵴保存术中利用PRF混合Bio⁃Oss®骨粉可以减少牙槽骨的吸收和减轻术后疼痛。
ObjectiveTo study the clinical outcomes of implanting platelet rich fibrinogen(PRF) mixed with Bio-Oss^■ in the extraction socket for alveolar ridge preservation and to provide evidence for clinical application.Methods Thirty-six patients who underwent alveolar ridge preservation were enrolled. Thirty-six extraction sites were divided intotwo groups: PRF mixed with Bio-Oss^■ group(test group) and Bio-Oss^■ alone(control group). Bone dimensional changesin height and width were measured by CBCT before and 6 months after surgery,and early soft tissue healing and postop-erative pain sensation were evaluated clinically 1 week after surgery.ResultsThere was no significant difference inthe alveolar bone height(-1.48 ± 0.40) mm between the test group and the control group. The difference in the alveolarbone width between the test group(-1.09 ± 0.42) mm and the control group(-1.35 ± 0.22) mm was statistically signifi-cant(z=-2.63,P=0.01). The postoperative pain score of the test group was 2.39 ± 1.20,and that of the control groupwas 3.39 ± 1.65,the difference was statistically significant(t=-2.083,P=0.045). There was no significant differencein soft tissue healing between the test group and the control group.ConclusionThe use of PRF mixed with Bio-Oss^■ in the alveolar ridge preservation procedure can reduce alveolar bone absorption and postoperative pain.
