超声光散射成像技术在早期乳腺癌诊断中的应用价值 被引量:8

摘要 目的探讨超声光散射成像技术(DOT)在早期乳腺癌临床诊断中的应用价值。方法采用回顾性研究方式,选取2017年5月至2018年12月于西安交通大学第二附属医院手术治疗并经病理检查确诊的乳腺肿块患者78例,均选择常规超声以及超声DOT检查,对比两种超声检查方式的诊断结果以及对于早期乳腺癌的诊断效能。结果78例患者的病理诊断结果中,良性23例,恶性55例;常规超声检查结果中,良性21例,恶性57例;超声DOT检查结果中,良性22例,恶性56例。以病理诊断结果为金标准,超声DOT检查的敏感度为94.65%、特异度为90.91%、阳性预测值为96.36%、阴性预测值为86.96%、诊断符合率为93.59%,常规超声检查的敏感度为85.96%、特异度为71.43%、阳性预测值为89.09%、阴性预测值为65.22%、诊断符合率为82.05%。超声DOT各项指标均高于常规超声,且特异度、阴性预测值、诊断符合率均明显高于常规超声,差异具有统计学意义(P<0.05)。结论超声DOT能够有效提高早期乳腺癌超声影像学诊断的准确率,可以有效减少误诊,同时重复性以及无创性较好,适用于早期乳腺癌的筛查诊断。 Objective To investigate the application value of ultrasonic light scattering imaging(DOT)in the clinical diagnosis of early breast cancer.Methods 78 patients with breast masses who were surgically treated and diagnosed by pathology at the Second Affiliated Hospital of Xi'an Jiaotong University from May 2017 to December 2018 were selected,retrospectively.Both routine ultrasound and ultrasound DOT were performed.The diagnostic results of ultrasonography and its diagnostic efficacy for early breast cancer were compared.Results Among the 78 patients,there were 23 benign tumors and 55 malignant tumors according to pathologic results.There were 57 cases of malignant tumors and 21 cases of benign tumors according to routine ultraound.The results of ultrasonic DOT imaging showed 56 cases of malignancy and 22 cases of benign.Using pathological diagnosis as the gold standard,the sensitivity,specificity,positive predictive value,negative predictive value and diagnostic coincidence rate of ultrasonic DOT examination were 94.65%,90.91%,96.36%,86.96%and 93.59%,respectively.The sensitivity,specificity,positive predictive value and negative predictive value of conventional ultrasound examination were 85.96%,71.43%,89.09%,65.22%and 82.05%,respectively.All the indicators of ultrasonic DOT were higher than those of conventional ultrasound,while the specificity,negative predictive value and diagnostic coincidence rate were significantly higher than those of conventional ultrasound,the difference was statistically significant(P<0.05).Conclusion Ultrasonic DOT imaging technology can effectively improve the accuracy of ultrasonic imaging diagnosis of early breast cancer,can effectively reduce misdiagnosis,while repeatability and non-invasive,suitable for early breast cancer screening and diagnosis.
