摘要
目的探讨腹膜后副神经节瘤和神经鞘瘤在多层螺旋CT上的影像表现及其鉴别价值.方法回顾性分析经病理证实的15例副神经节瘤和17例神经鞘瘤的一般资料(年龄,性别)和MSCT表现(病灶大小、形态、密度、边界、均匀度、钙化、囊变/坏死、囊变/坏死分布位置、出血、强化程度),对比分析两种肿瘤的临床和MSCT表现差异.结果副神经节瘤组的囊变/坏死分布位置以周边为主,神经鞘瘤组以中心囊变坏死多见,两组间差异有统计学意义(P<0.05);副神经节瘤组实质的平扫CT值高于神经鞘瘤组(P<0.01),动、静脉期强化值、动脉期强化率均有统计学差异(P均<0.01);副神经节瘤的强化模式?"速升缓降"型,而神经鞘瘤呈现为"渐升"型.结论病灶内囊变/坏死分布位置,平扫密度,动、静脉期强化值,动脉期强化率,强化模式有助于腹膜后副神经节瘤和神经鞘瘤的鉴别诊断.
Objective To evaluate and compare the MSCT features of retroperitoneal paragangliomas and schwannomas for obtaining valuable diagnosis and differentiation information of these two tumors.Methods The demographic information and CT features of 15 cases of paragangliomas were retrospectively reviewed and 17 cases of schwannomas which were confirmed by pathology and these information and CT features were compared between paragangliomas and schwannomas by using t test and Fisher test.Results In paraganglioma group,cystic degeneration and necrosis were predominantly located in the periphery,while in schwannoma group,central cystic degeneration and necrosis were more common,which showed statistically significant differences(P<0.05).The plain CT value of the tumor in the paraganglioma group was higher than that in schwannoma group,and the difference was statistically significant(P<0.01).There were significant differences in CT value both in arterial,venous phase and arterial phase enhancement rate between the two groups(P<0.01).Paraganglioma showed a enhancement pattern of“fast wash-in and slow wash-out”,while schwannoma showed gradually persistent enhancement.Conclusion Cystic degeneration and necrosis are predominantly located in the periphery and higher density on plain scan are more common in paraganglioma than in schwannoma.Meanwhile the enhancement pattern of"fast wash-in and slow wash-out"in paraganglioma is different from the gradually persistent enhancement in schwannoma which will help to differentiate both diseases.
