摘要
目的探讨妊娠期高血压疾病(HDP)患者中医证候与体质分布、循环MicroRNA的研究,为中医诊治HDP提供重要的参考依据.方法收集298例HDP患者的中医临床症状及舌苔脉象,判定中医证候与体质的类型,另选30例正常妊娠孕妇设为正常对照组,采用实时荧光定量PCR方法检测各组血清miR-885-5p、miR-26a-5p、miR-155、miR-210表达的情况.结果HDP患者中医证型主要分为脾虚肝旺型、阴虚肝旺型、肾虚水泛型、脾虚湿盛型;中医体质以气虚质为主,其次为阴虚质、阳虚质、痰湿质及湿热质.两者之间存在相似性和矛盾性.HDP外周血中检测到的miR-885-5p及miR-26a-5p分别在阴虚肝旺组和脾虚肝旺组表达水平高于其他证型组(P<0.001),而肾虚水泛型患者血液中检测到的?miR-155、miR-210表达水平高于其他证型组(P<0.001).结论HDP患者中医证型以脾虚肝旺型为主,中医体质以气虚质为主,证候与体质之间关系密切.脾虚肝旺型、阴虚肝旺型、肾虚水泛型、脾虚湿盛型之间存在明显miR-885-5p、miR-26a-5p、miR-155、miR-210表达差异,HDP不同中医证型临床表现与MicroRNA失调存在相关性.
Objective To investigate the distribution of TCM syndromes,constitution and circulating microRNA in patients with Hypertensive disorder of pregnancy(HDP),so as to provide important reference for the clinic diagnosis and treatment of HDP in TCM.Methods Clinical symptoms and tongue fur pulse of 298 HDP patients were collected to determine the type of TCM syndromes and constitution.Another 30 normal pregnant women were selected as normal control group.The expression of serum microRNA-885-5p,microRNA-26a-5p,microRNA-155 and microRNA-210 in each group were detected by real-time fluorescence quantitative PCR.Results The TCM syndromes of HDP patients were mainly divided into Insufficiency of the spleen with overabundance of dampenss type,spleen deficiency and liver hyperacriveity type,hyperactiveity of liver fire due to yin deficiency type,edema due to the kidney insufficiency type.The constitution of TCM was mainly Qi deficiency,followed by Yin deficiency,Yang deficiency,phlegm-dampness and dampness-heat.There were similarities and contradictions between them.The levek of microRNAs-885-5p and microRNAs-26a-5p detected in peripheral blood of HDP patients were higher in hyperactiveity of liver fire due to yin deficiency type and spleen deficiency and liver hyperacriveity type than in other syndromes group(P<0.001),while the levels of microRNAs-155 and microRNAs-210 detected in blood of edema due to the kidney insufficiency type patients were higher than those in other syndromes group(P<0.001).Conclusion This study found that the main TCM syndromes of HDP patients are spleen with overabundance of dampenss type,and the constitution of TCM is Qi deficiency.There is a close relationship between the syndromes and constitution.There are significant differences in the expression of microRNA between spleen with overabundance of dampenss type,spleen deficiency and liver hyperactiveity type,hyperactiveity of liver fire due to yin deficiency type,edema due to the kidney insufficiency type.The clinical manifestations of different TC
