摘要
目的:比较不同手术入路对全髋关节置换术患者髋臼假体位置的影响。方法:回顾性分析我院于收治的68例人工全髋关节置换术患者,按治疗方式的不同分为A、B组,各34例,均行人工全髋关节置换术治疗,A组直接前入路,B组后外侧入路。比较两组患者手术、术后48 h髋臼假体位置及术后并发症情况。比较两组术前、术后48 h炎性因子水平、疼痛程度。结果:A组切口长度、出血量、下床时间及住院时间均显著少于或短于B组(P<0.05)。与术前相比,术后48h两组血清IL-1、CRP均较治疗前升高,且A组低于B组(P<0.05)。两组术后48 h内VAS评分随时间的延长呈下降趋势,A组显著低于B组(P<0.05)。两组并发症发生率差异不显著(P>0.05)。结论:直接前入路行全髋关节置换术效果显著,改善患者疼痛程度及炎性因子水平,利于患者预后恢复。
Objective: The effects of different surgical approaches on the position of acetabular prostheses in patients undergoing total hip arthroplasty were compared. Methods: A retrospective analysis was performed on 68 patients undergoing total hip arthroplasty in our hospital. They were divided into groups A and B according to different treatment methods, with 34 cases in each group. All patients underwent total hip arthroplasty., Group B posterolateral approach. The acetabular prosthesis position and postoperative complications were compared between the two groups. The levels of inflammatory factors and pain were compared between the two groups before and after 48 hours. Results: The length of incision, bleeding volume, time to get out of bed, and length of stay in group A were significantly shorter or shorter than those in group B(P<0.05). Compared with before surgery, the serum levels of IL-1 and CRP in both groups were higher than those before treatment at 48 h after surgery, and group A was lower than that in group B(P<0.05). Within two hours after operation, the VAS score of the two groups showed a downward trend with time, and group A was significantly lower than group B(P<0.05). There was no significant difference in the incidence of complications between the two groups(P>0.05). Conclusion: Direct anterior approach for total hip arthroplasty has significant effects, improves patient pain and inflammatory factor levels, and is beneficial to patients’ prognosis.
