摘要
目的探讨紧密连接蛋白的闭合蛋白(Occludin)和紧密粘连蛋白-1(ZO-1)在不同程度脾破裂大鼠血清及回肠黏膜中的表达及意义。方法将48只健康大鼠随机分成脾破裂轻度组、中度组、重度组和假手术组(Sham组)4组,每组12只,前3组采用开腹手术方式制作不同程度脾破裂大鼠模型,Sham组仅接受开、关腹手术。观测大鼠腹腔内情况;ELISA法检测各组大鼠血清Occludin和ZO-1的含量;HE染色法观察回肠黏膜组织病理学改变;免疫组化染色法检测回肠黏膜组织中Occludin及ZO-1的表达情况[以光密度(OD值)表示]。结果 Sham组腹腔内无渗出及出血;轻度组腹腔内平均约1.6ml积血,脾脏创面少许渗出;中度组腹腔内平均约3.5ml积血,肠管颜色较苍白,脾脏创面渗出较明显;重度组腹腔内平均约4.3ml积血,肠管苍白明显,肠管间部分粘连,脾脏创面渗出明显。病理学检查显示中度组和重度组大鼠的回肠黏膜损伤较Sham组及轻度组明显为重。与Sham组相比,中、重度组血清中Occludin及ZO-1含量、回肠黏膜组织中Occludin及ZO-1的OD值均有所升高或降低(均P<0.05);与轻度组相比,中、重度组血清中Occludin及ZO-1含量、回肠黏膜组织中Occludin及ZO-1的OD值均有所升高或降低(均P<0.05)。结论血清中Occludin及ZO-1含量增高可作为临床上观测脾破裂及肠黏膜损伤程度的一个可靠指标。
Objective To investigate the expression and significance of tight-junction Occludin and ZO-1 expression in ileum mucosa and serum in rats with different degree rupture of spleen. Methods Forty-eight male rats were divided into four groups: sham group, mild spleen rupture group, medium spleen rupture group, severe spleen rupture group randomly with 12 rats in each group. The spleen rupture was induced by laparotomy in three spleen rupture groups, while the open-close laparotomy was performed in sham group. The intraperitoneal conditions of rats were observed;the serum levels of Occludin and ZO-1 were detected by ELISA;the expression levels of Occludin and ZO-1 in ileum mucosa were detected by immunohistochemistry staining and denoted by optical density value(OD value). Results There was no exudation or hemoperitoneum in sham group. The average amount of blood of the mild group was 1.6 ml, and there was a little exudation on the spleen wound surface. In the medium group, the average hemoperitoneum was 3.5 ml, the color of the intestine was relatively pale, and the spleen wound exudation was obvious. In the severe group, the average hemoperitoneum was 4.3 ml, the color of the intestine was very pale with partial adherence, the spleen wound exudation was more obvious. Pathological examination showed that the ileum mucosal damage in the medium and severe groups was significantly more serious than that in the sham and mild groups. Compared with the sham group and mild group, the contents of Occludin and ZO-1 in serum were increased and the OD values of Occludin and ZO-1 in ileal mucosal tissues were decreased in the medium and severe groups(all P<0.05). Conclusion The increased contents of Occludin and ZO-1 in serum might be used as an indicator for the degree of spleen rupture and intestinal mucosal damage.
