系统评价药师主导的药物重整对急诊科的影响 被引量:3

Systematic review of the impact of pharmacist-led medication reconciliation on emergency department

摘要 目的通过系统评价药师主导的药物重整对急诊科的影响,以评估药师服务的益处。方法计算机检索PubMed、EMbase、The Cochrane、中国生物医学文献数据库、万方数据库和中国知网数据库,搜集有关药师主导的药物重整对急诊科常规护理(医护人员)影响的研究,检索时限均从建库至2019年9月。试验组为药师主导的药物重整,对照组为常规医护组。用RevMan 5.3软件对药疗偏差患者比例、药疗偏差数量、人均药疗偏差数量主要指标进行Meta分析。结果共纳入10篇研究,包括6656例急诊患者,其中试验组5561例,对照组1095例。Meta分析结果显示,试验组的药疗偏差患者比例(29.4%vs. 64.4%),药疗偏差数量(10.1%vs. 17.8%),人均药疗偏差数量(WMD=-2.31,95%CI=-3.68~0.94)均较对照组显著降低(均P<0.05)。其中,药疗偏差最常见是药物遗漏(59%),剂量/频率不正确或遗漏(25%)。结论药师主导的药物重整可以显著降低急诊患者的药疗偏差,体现了药师服务的价值。 Objective To evaluate the impact of pharmacist-led drug reformation on emergency department to evaluate the benefits of pharmacist services.Methods Computers were searched for PubMed, EMbase, The Cochrane, CBM, Wanfang Data and CNKI databases to collect research on the effects of pharmacist-led drug reforming on routine care(medical staff) in the emergency department.The search time was from the establishment of the library to September 2019.Meta-analysis was performed with RevMan 5.3 software on the main indicators of the proportion of patients with drug therapy deviation, the number of drug therapy deviations, and the number of drug therapy deviations per capita.Results A total of 10 studies were included, including 6656 emergency patients.There were 5561 cases in the treatment group and 1095 cases in the control group.The results of the meta-analysis showed that compared with control group, the patients with medication deviation(29.4% vs.64.4%), the number of medication deviations(10.1% vs.17.8%), the number of deviations per capita medication(WMD=-2.31, 95%CI=-3.68--0.94) in treatment group were significantly reduced(all P<0.05).Among them, the most common deviation of medication is drug omission(59%), the dose/frequency is incorrect or missing(25%).Conclusion Pharmacist-led drug reformation can significantly reduce the deviation of medications in emergency patients, reflecting the value of pharmacist services.
