盆痛灵灌肠对子宫内膜异位症模型大鼠疼痛行为的影响及其机理探讨 被引量:6

摘要 目的:观察盆痛灵直肠给药对子宫内膜异位症模型大鼠疼痛行为的影响作用及其作用机理探讨。方法:采用自体内膜移植法建立SD大鼠EMs模型,将造模成功的大鼠随机分为模型组、消炎痛组和盆痛灵高、中、低剂量组。另设假手术组(仅切除单侧结扎段子宫)。采用VonFrey2390电子测痛仪测量大鼠机械压力痛阈,观察给药前后不同时间点大鼠痛阈值的变化以及内异灶体积的变化,ELISA检测EMs大鼠血清及腹腔液中NGF、IL-1β和TNF-α的浓度,评价盆痛灵方的镇痛作用及其机理。结果:模型组大鼠内异灶体积随着时间变化而增大,而大鼠的机械痛阈值则降低。盆痛灵方直肠给药后,盆痛灵各组内异灶体积均有所缩小,同时盆痛灵各组大鼠机械痛阈值上升。ELISA结果显示,与假手术组比较,模型组血清和腹腔液中的NGF、IL-1β和TNF-α浓度均升高。与模型组比较,西药和盆痛灵高、中剂量组血清及腹腔液中IL-1β、NGF和TNF-α水平均降低,盆痛灵低剂量组血清中IL-1β降低。结论:内异症SD大鼠体内自体移植的内异灶体积与大鼠机械疼痛阈值成负相关,盆痛灵直肠给药可缩小大鼠内异灶体积,提高SD大鼠的机械痛阈值,且盆痛灵剂量浓度与镇痛效果成正相关,其作用可能与其降低大鼠血清及腹腔液NGF、IL-1β和TNF-α水平相关。 Objective:To observe the effect of Pentongling Enema on pain behavior,and to explore its action mechanism in the model rats of endometriosis(EMs).Methods:The EMs model was established by autotransplantation of endometrium in SD rats.After successful modeling,the rats were randomly divided into the model group,the Indomethacin group and the Pentongling groups of high-dose,medium-dose and low-dose.Rats with unilateral hysterectomy were included into the sham-operation group.The mechanical pressure pain threshold was measured by VonFrey2390 pain threshold detector.The changes of pain threshold and endometriotic lesions were observed at different time points before and after medication.The levels of NGF,IL-1β and TNF-α in serum and in peritoneal fluid were detected by Elisamethod.The analgesic effect of Pentongling Enema was analyzed.Results:The lesion size of the model rats increased with time,and the mechanical pain threshold decreased.After the intervention of Pentongling Enema,the lesion size decreased,and the mechanical pain threshold increased.The results of ELISA showed that the levels of NGF,IL-1β and TNF-α in serum and in peritoneal fluid increased in the model group compared to those in the sham-operation group;the levels of NGF,IL-1β and TNF-α decreased in the Indomethacin group and in the Pentongling group of high-dose and medium-dose compared to those in the model group;however,the Pentongling group of high-dose only showed decrease in IL-1β.Conclusion:The lesion volume of autotransplantation endometrium was negatively correlated with the mechanical pain threshold in EMs rats.The administration of Pentongling Enema can reduce the volume of the internal heterotopic lesion and improve the mechanical pain threshold.The dose concentration of Pentongling Enema is positively correlated with the analgesic effect.The mechanism may be related to the reductions of NGF,IL-1β and TNF-α in serum and peritoneal fluid.
