摘要
目的评价复方电解质注射液(Ⅱ)简称MEI-Ⅱ,及其应用于围术期患者液体治疗的效果。方法计算机检索PubMed,Embase,The Cochrane Library,CNKI,VIP,WanFang Data数据库,检索时限为自建库至2019年6月,对纳入研究进行数据提取和偏倚风险评价,采用Revman 3.5软件对评价结果进行定量分析。结果共纳入7个研究。Meta分析结果显示,酸碱平衡方面,与对照组比较,MEI-Ⅱ组全血pH差异无统计学意义,HCO3-水平较高[MD=1.20,95%CI(0.58,1.81),P=0.0001],全血碱剩余(BE)水平较高[MD=1.72,95%CI(1.22,2.23),P<0.00001],血乳酸水平较低[MD=-0.64,95%CI(-1.25,-0.02),P=0.04],差异均有统计学意义;电解质平衡方面,与对照组比较,MEI-Ⅱ组血清氯水平较低[MD=-2.13,95%CI(-4.20,-0.05),P=0.04],血清Ca2+较高[MD=0.17,95%CI(0.04,0.31),P=0.01],血清Mg2+较高[MD=0.19,95%CI(0.16,0.23),P<0.00001],差异均有统计学意义;两组患者血清Na+和K+水平变化比较,差异均无统计学意义。结论MEI-Ⅱ在维持酸碱平衡及维持血清Cl-,Ca2+,Mg2+水平稳定方面效果较其他晶体注射液显著,适合用于围术期液体治疗。
Objective To systematically evaluate the effect of multiple electrolyte injection(MEI-Ⅱ)on the acid-base balance and electrolytes in patients with perioperative fluid therapy.Methods Randomized controlled trials(RCTs)concerning MEI-Ⅱprescribed in the perioperative period were retrieved in English databases such as Pubmed,EMBase,the Cochrane Library and Chinese databases such as CNKI,and WanFang Data.The retrieval time was from the inception of the databases to June 2019.Results A total of 7 studies were included.The results of meta-analysis showed that in terms of acid-base balance,the blood pH in the MEI-Ⅱgroup the control group had no significant difference,t he HCO3-level was higher[MD=1.20,95%CI(0.58,1.81),P=0.0001],base excess(BE)level was higher[MD=1.72,95%CI(1.22,2.23),P<0.00001],and lactate level was lower[MD=-0.64,95%CI(-1.25,-0.02),P=0.04].The differences were all statistically significant.In terms of electrolyte balance,the MEI-Ⅱhad lower serum chlorine levels than the control group[MD=-2.13,95%CI(-4.20,-0.05),P=0.04],serum Ca2+was higher[MD=0.17,95%CI(0.04,0.31),P=0.01],and serum Mg2+was higher[MD=0.19,95%CI(0.16,0.23),P<0.00001],the differences were also statistically significant.There was no significant difference in the effects of serum Na+levels and K+levels.Conclusion MEI-Ⅱis more effective than other crystalloids in maintaining acid-base balance and maintaining serum Cl-,Ca2+,Mg2+levels,which is suitable for perioperative fluid therapy.
