Expression levels and clinical significances of serum LCN-1 and LCN-2 in COPD patients at different stages

摘要 目的探讨慢性阻塞性肺疾病(COPD)患者不同时期血清脂质运载蛋白-1(LCN-1)、脂质运载蛋白-2(LCN-2)的表达水平及临床意义。方法选取2016年9月-2018年9月在新乡市第一人民医院接受治疗的COPD患者150例,其中急性加重期患者85例,稳定期患者65例,另选取同期来院体检的健康人员50例作为对照组;采集受试者血液收集血清,检测白细胞(WBC)和中性粒细胞百分比(NEUT%),肺功能仪测定肺功能指标,ELISA检测血清LCN-1、LCN-2、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子(TNF-α)、白细胞介素-8(IL-8)表达水平,Pearson相关系数法分析LCN-1与LCN-2的相关性及二者与临床指标的相关性,ROC曲线分析LCN-1与LCN-2的临床诊断价值,对影响COPD的多因素进行Logistic回归分析。结果稳定期组和急性加重期组FEV1%pred、第1秒用力呼气容积/用力肺活量(FEV1/FVC)显著低于对照组,且急性加重期组较稳定期组低,而WBC、NEUT%、LCN-1、LCN-2、hs-CRP、TNF-α、IL-8显著高于对照组,且急性加重期组较稳定期组高,差异有统计学意义(P<0.05);LCN-1、LCN-2表达水平与FEV1%、FEV1/FVC呈显著负相关,与WBC、NEUT%、hs-CRP、TNF-α、IL-8均呈显著正相关(P<0.05);LCN-1与LCN-2呈显著正相关(r=0.923,P=0.000);LCN-1、LCN-2均可作为COPD的临床诊断指标;LCN-1、LCN-2、WBC、NEUT%、TNF-α、IL-8、hs-CRP均是影响COPD病情的危险因素,FEV1%、FEV1/FVC是COPD病情的保护因素。结论 LCN-1、LCN-2在COPD患者血清中表达水平显著升高,且随病情加重逐渐升高,与COPD患者病情具有密切关系,均可作为COPD的临床诊断指标。 Objective To investigate the expression levels and clinical significances of serum lipocalin-1(LCN-1)and lipocalin-2(LCN-2)in patients with chronic obstructive pulmonary disease(COPD)at different stages. Methods From September 2016 to September 2018,150 patients with COPD were selected,including 85 patients at acute exacerbation stage and 65 patients at stable stage;another 50 healthy persons who came to the hospital for physical examination during the same period were selected as control group. Blood samples were collected from subjects;the leucocyte count and neutrophil percentage were detected. Lung function indexes were measured by lung function instrument. Levels of serum LCN-1,LCN-2,highly sensitive C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α)and interleukin-8(IL-8)were detected by ELISA. Pearson correlation coefficient method was used to analyze the correlation between LCN-1 and LCN-2 and their correlations with clinical indicators. ROC curve was used to analyze the clinical diagnostic values of LCN-1 and LCN-2,Logistic regression analysis was performed on multiple factors affecting COPD. Results Forced expiratory volume in one second(FEV1)/forced vital capacity(FVC)in stable group and acute exacerbation group were significantly lower than those in control group,and that of the acute exacerbation group was lower than that in stable group,while the white blood cell count,neutrophil percentage,LCN-1,LCN-2,hs-CRP,TNF-α,IL-8 levels were significantly higher than those in control group,and those of the acute exacerbation group was higher than those in stable group(P<0.05). The expression levels of LCN-1 and LCN-2 were significantly negatively correlated with FEV1% and FEV1/FVC,and were significantly positively correlated with white blood cell count,neutrophil percentage,hs-CRP,TNF-α and IL-8(P<0.05).
