前方牵引联合快速扩弓治疗替牙期骨性Ⅲ类错[牙合]畸形的疗效分析被引量：9

Effect analysis of maxillary protraction combined with rapid maxillary expansion in the treatment of type Ⅲ mixed dentition

摘要目的研究前方牵引联合快速扩弓治疗替牙期骨性Ⅲ类错牙合畸形疗效。方法回顾分析我院2016年11月~2018年3月收治的替牙期骨性Ⅲ类错牙合畸形患者51例作为研究对象,均采用前方牵引联合快速扩弓治疗,测量矫正前后硬组织指标、上气道指标与面部美学指标。结果与矫正前比,矫正后硬组织指标SNA,U1-NA距,U1-NA L1-NB增加,SNB、L1-NB距减小;上气通指标:PNS-UPW,PNS-Ad2,OAW2,OAW3,SPP-SPPW,U-MPW,V-LPW增加,OAW1减小;面部美学指标:PNS-UPW,PNS-Ad2,OAW2,OAW3,SPP-SPPW,U-MPW,V-LPW增加,OAW1减小,P<0.05差异具有统计学意义。结论对替牙期骨性Ⅲ类错牙合畸形患者采用前方牵引联合快速扩弓治疗可纠正反牙合,改善患者Ⅲ类面型,增加上气道宽度。 Objective To study the effect of maxillary protraction combined with rapid maxillary expansion in the treatment of type Ⅲ orthodontic deformities in mixed dentition. Methods 51 patients with osteostomy type Ⅲ malocclusion in the orthodontic phase from November 2016 to March 2018 in our hospital were reviewed and analyzed.All patients were treated with maxillary protraction combined with rapid maxillary expansion. The results of cephalometric analysis, upper airway index and facial aesthetic index were measured. Results After orthodontic treatment, SNA angle increased, SNB angle decreased, U1-NA distance increased, U1-NA angle increased, L1-NB distance decreased, and L1-NB angle increased. After orthodontic treatment PNS-UPW is high, PNS-Ad2 is high, OAW1 is low, OAW2 is high,OAW3 is high, SPP-SPPWW is high, U-MPW is high, and V-LPW is high;After treatment, PNS-UPW is high, PNSAd2 is high, OAW1 is low, OAW2 is high, OAW3 is high, SPP-SPPWW is high, U-MPW is high, and V-LPW is high.P <0.05(with statistical differences). Conclusion It is worth reference to apply maxillary protraction combined with rapid maxillary expansion therapy to patients with osteoplastic type Ⅲ malocdusion in mixed dentition to improve the facial aesthetics.
