摘要
目的探讨床旁彩色多普勒超声在诊断新生儿颅脑疾病中的应用价值。方法研究纳入该院新生儿监护病房(NICU)收治的160例存在颅脑疾病高危因素的新生儿,病例资料选取时间从2017年1月—2019年1月,均接受床旁彩色多普勒超声及CT检查,对比和分析检查结果。结果彩色多普勒超声检测中新生儿的脉络丛及室管膜下囊肿、局灶性室管膜下出血、蛛网膜及硬脑膜下出血、Ⅰ级颅内出血等的检出率(12.50%、46.87%、11.87%、63.75%)均高于CT检查(2.50%、5.00%、5.00%、30.62%),组间对比差异有统计学意义(χ2=11.531、73.025、4.894、63.212,P<0.05)。而彩色多普勒超声与CT检查中新生儿的脑实质内出血、Ⅱ级颅内出血、缺氧缺血性脑病、Ⅲ级颅内出血、Ⅳ级颅内出血等检出率相比较差异无统计学意义(χ2=0.114、0.069、0.094、0.203、0.146,P>0.05)。结论彩色多普勒超声在新生儿颅脑疾病诊断中有较高的应用价值,尤其是对脉络丛囊肿、室管膜下囊肿、蛛网膜及硬脑膜下出血、Ⅰ级颅内出血等具有较高的检出率,因此可为临床诊断疾病提供重要的应用价值。
Objective To explore the value of bedside color Doppler ultrasound in the diagnosis of craniocerebral diseases in newborns.Methods A total of 160 neonates with high risk factors for craniocerebral disease were enrolled in the neonatal care unit(NICU)of the hospital.The case data were collected from January 2017 to January 2019,and all received bedside color Doppler ultrasound and CT examination,comparison and analysis of inspection results.Results The detection rates of neonatal choroid plexus and subventricular cysts,focal subventricular hemorrhage,arachnoid and subdural hemorrhage,gradeⅠintracranial hemorrhage(12.50%,46.87%,11.87%,63.75%)were higher in color Doppler ultrasound detection than CT examination(2.50%,5.00%,5.00%,30.62%),the contrast difference between the groups was statistically significant(χ2=11.531,73.025,4.894,63.212,P<0.05).The detection rates of neonatal cerebral parenchymal hemorrhage,gradeⅡintracranial hemorrhage,hypoxic ischemic encephalopathy,gradeⅢintracranial hemorrhage,gradeⅣintracranial hemorrhage and so on,the difference was not statistically significant(χ2=0.114,0.069,0.094,0.203,0.146,P>0.05).Conclusion Color Doppler ultrasound has higher application value in the diagnosis of neonatal craniocerebral diseases,especially for choroid plexus cysts,subventricular cysts,arachnoid and subdural hemorrhage,gradeⅠintracranial hemorrhage,etc.The detection rate can provide important application value for clinical diagnosis of diseases.
