摘要
目的:观察腰椎管狭窄症患者X线片、MR片的相关内容变化,探讨腰椎管狭窄症患者的腰大肌面积情况,研究其与其他相关影像学结果的相关性,从而探讨更合理、更便捷的治疗方法。方法:通过回顾269例患者的站立位腰椎正侧位X线片和腰椎MR片,最终确定为腰椎管狭窄症者89例,观察其性别、年龄、弓形面积、腰大肌最大横截面积的情况,探讨其相关性。结果:89例腰椎管狭窄症患者腰大肌横截面积与腰椎弓形面积呈正相关性,而与年龄、硬膜囊和马尾神经受压分级均呈负相关性。且腰椎弓形面积与硬膜囊和马尾神经受压分级的关系也是呈负相关性改变。结论:腰椎管狭窄症患者的腰大肌维系腰椎生理曲度的力量减弱,当生理曲度变小时,椎管容积也随之变小,脊髓压迫也更加明显,故合理功能锻炼是关键。
Objective: To observe the changes in X-ray and MR images of patients with lumbar spinal stenosis,and to investigate the maximus area of Psoas major in patients w ith lumbar spinal stenosis,and to study the correlation w ith other relevant imaging,so as to explore more reasonable and convenient treatment methods. Methods: A total of 89 patients w ith lumbar spinal stenosis w ere identified by a review of the standing lumbar frontal and lateral X-ray and lumbar M R images of 269 patients.,and the sex,age,Arch form area and maximal cross-sectional area of the psoas major w ere observed. Results: in 89 patients w ith lumbar spinal stenosis,the cross-sectional area of the psoas major w as positively correlated w ith the lumbar Arch form area,but negatively correlated w ith age,dural sac,and cauda equina nerve compression grade. There w as also a negative correlation betw een the lumbar Arch form area and the grade of dural sac and cauda equina nerve compression. Conclusion: In patients w ith lumbar spinal stenosis,the strength of the psoas major muscle to maintain the physiological curvature of the lumbar spine is w eakened. When the physiological curvature becomes w eak,the lumbar Arch form area becomes smaller and the spinal cord compression becomes more obvious. Therefore,reasonable functional exercise is the key.
