脐静脉置管在超低/极低出生体重儿全静脉营养中的应用研究 被引量:1

摘要 目的探讨脐静脉置管在超低/极低出生体重儿全静脉营养中的应用价值。方法选取2015年8月3日至2017年12月31日间我院NICU收治的体重<1500 g的早产儿302例,根据输液治疗途径不同,分为研究组(脐静脉置管组)200例和对照组(外周静脉留置针组)102例。比较两组的生理性体重下降恢复时间、导管留置时间、住院时间、导管合并症等情况。结果研究组的生理性体重下降恢复时间、住院时间,导管留置时间均优于对照组,两组比较差异均有统计学意义(均P<0.05);UVC合并症中腹胀发生率最高,导管感染菌以大肠埃希菌感染占比例最高。结论 UVC可解决超未成熟儿、超低/极低出生体重儿输液困难、反复穿刺、静脉营养用药等问题,缩短住院时间,减少医疗费用,促进疾病康复,但行UVC同时导管感染问题不能忽视。 Objective To explore the application value of umbilical vein catheterization in total parenteral nutrition for extremely/very low birth weight infants. Methods A total of 302 preterm infants with weight of less than 1500 g who were treated in our NICU from August 3,2015 to December31, 2017 were selected and divided into a study group(umbilical vein catheterization group)of 200 cases and the control group(peripheral vein indwelling needle group)of 102 cases according to different infusion treatment routes. The physiological recovery time, catheter indwelling time, hospital stay time, and catheter complications were compared between the two groups. Results The recovery time of physiological weight, the hospital stay time and the umbilical vein indwelling time in study group were better than those in the control group, the difference between the two groups was statistically significant(P<0.05). The incidence of abdominal distension was highest in UVC comorbidities, and catheter infection. Escherichia coli infections accounted for the highest proportion. Conclusion UVC can solve the problems of difficulty in infusion, repeated puncture, intravenous nutrition, etc. in ultra-immature infants, extremely/very low birth weight infants, shorten hospital stay, reduce medical costs, and promote disease recovery. However, it shouldn’t be ignored of the catheter infection of UVC.
